摘要
目的:探讨持续封闭负压引流(VSD)技术在治疗急性坏死性筋膜炎(NF)中的应用价值。方法:将医院收治的15例急性坏死性筋膜炎患者根据治疗方法的不同分为观察组(10例)和对照组(5例),观察组采用持续封闭负压引流术治疗,对照组采用传统处理方式进行治疗;对比分析两组的治疗效果,并应用生活质量SF-36量表评价两组患者8个维度的得分,分析VSD的临床应用价值。结果:观察组一次植皮后创面愈合比例为100%,明显高于对照组的60%,其差异有统计学意义(x2=4.615,P<0.05);观察组的平均愈合时间为(16.53±9.25)d,明显低于对照组的(26.35±7.16)d,其差异有统计学意义(t=32.637,P<0.05)。观察组患者SF-36量表中生理功能(PF)、生理职能(RP)、躯体疼痛(BP)、总体健康(GH)、活力(VT)和精神健康(MH)的得分明显高于对照组,其差异有统计学意义(t=3.74,t=3.07,t=2.62,t=4.98,t=3.55,t=2.57;P<0.05);两组患者的社会功能(SF)和情感职能(RE)得分差异无统计学意义(t=0.77,t=0.68;P>0.05)。结论:利用持续VSD技术治疗NF具有促进肉芽生长、促进创面愈合、降低创面感染率和缩短病程等作用,临床疗效显著,并可提高患者的生活质量。
Objective:To investigate the application value of vacuum-sealing drainage(VSD)in the treatment of necrotizing fasciitis(NF).Methods:15patients with acute NF were divided into observation group(n=10cases)and control group(n=5cases).The patients of observation group underwent the treatment of VSD while the patients of control group underwent the treatment of routine method.The curative effects of the two group were compared,and SF-36scale of life quality were applied to evaluate the scores of two groups in8aspects,and then the clinical value of VSD was analyzed as above results.Results:After a skin grafting,the proportion of wound healing of observation group(100%)was significantly higher than that of control group(60%)(x2=4.615,P﹤0.05).The average healing time of observation group(16.53d±9.25)was significantly lower than that of control group(26.35d±7.16)(t=32.637,P<0.05).In the SF-36scale,the scores of PF,RP,BP,GH,VT and MH of observation group were significantly higher than that of control group(t=3.74,t=3.07,t=2.62,t=4.98,t=3.55,t=2.57,P<0.05),respectively.While the differences of SF and RE between the two groups were no statistical significance(t=0.77,t=0.68,P>0.05),respectively.Conclusion:The VSD technique is applied to treat NF has series of advantages included of promoting the growth of granulation tissue and the healing of wound,and reducing wound infection,and shortening the course of disease,etc.Therefore,it has significant curative effect,and can enhance the life quality of patient.
