摘要
目的分析乳腺癌乳腔镜腋窝淋巴结清扫术(EALND)中保留肋间臂神经(ICBN)以及胸前神经(ATN)的临床效果。方法选取唐山市协和医院80例采用乳腔镜腋窝淋巴结清扫术治疗的乳腺癌患者作为研究对象,根据其是否保留了ICBN和ATN将其分为观察组(保留ICBN和ATN)和对照组(不保留ICBN和ATN),观察组43例,对照组37例。对比2组各项临床指标。结果观察组患者的感觉异常发生率和并发症发生明显低于对照组(P<0.05);2组患者的手术时间相比差异无统计学意义,观察组治疗后的生活质量评分、VAS评分和皮肤异常评分均明显优于对照组(P<0.05);观察组患者的胸肌萎缩发生率和外形改变发生率均低于对照组(P<0.05)。结论乳腺癌行EALND保留ICBN以及ATN具有较高的安全性,且有利于降低患者的胸肌萎缩发生率和外形改变发生率。
Objective To analyze clinical effects on the preservation of intercostobrachial nerve(ICBN)and anterioresthoracales nerve(ATN)during endoscopic axillary lymph node dissection(EALND)of breast cancer are analyzed.Methods80casesbreast cancerwithaxillary lymph node dissectionin Union Hospital of Tangshan City were divided into observation group and control group according toretention of intercostobrachial nerve and thoracic nerve,43cases in observation group,37cases in control group.Compared the clinical indicators between two groups.Results The incidence and complication of sensory abnormalities in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the operation time between the two groups.The quality of life,VAS scores anddermal abnormalities scores were better than that of control group(P<0.05).The incidence of thoracic atrophy and the change of shape of the patients were significantly lower than those of the control group(P<0.05).Conclusion The preservation of ICBN and ATN during endoscopic axillary lymph node dissection(EALND)of breast cancer has high security.Meanwhile,it is helpful to reduce the incidence for chest muscle atrophy and shape change of the patients.
