摘要
目的探讨乳腺癌手术中纳米碳混悬注射液淋巴示踪对淋巴结清扫及神经保护的临床意义。方法回顾性分析112例乳腺癌患者手术资料,将注射纳米碳混悬注射液并保留肋间臂神经的56例作为研究组,常规手术的56例作为对照组,比较2组淋巴结检获数目以及术后感觉障碍发生情况。结果 (1)前哨淋巴结:研究组103枚,平均每例(1.84±1.1)枚,对照组76枚,平均每例(1.35±1.4)枚;(2)微小淋巴结:研究组检获456枚,平均每例(8.1±1.2)枚,对照组检获淋巴结218枚,平均每例(4.2±2.3)枚;(3)淋巴结总数:研究组1847枚,平均每例(26.5±3.2)枚,对照组1289枚,平均每例(15.2±5.3)枚;(4)术后感觉障碍发生率:研究组为12.5%(7/56),对照组为67.8%(38/56)。组间比较差异均有统计学意义(P<0.05)。结论乳腺癌手术中纳米碳混悬注射液淋巴示踪,有助于提高淋巴结检获率,同时通过负显影,有助于更好地保留肋间臂神经。
Objective To investigate the clinical significance of carbon nanoparticles tracing in lymph node dissection and neuroprotection in breast cancer surgery.Methods The surgical data of 112 patients with breast cancer were studied.56 patients with injection of nano carbon suspension and reserved intercostobachial nerves were regarded as the study group,and 56 patients with conventional operation as the control group.The number of lymph nodes and the occurrence of postoperative sensory disturbances in the two groups were compared retrospectively.Results(1)Sentinel lymph node:In study group 103 sentinel lymph nodes were detected,with an average of(1.84±1.1)per case,and 76 sentinel lymph nodes were detected in control group,with an average of(1.35±1.4)per case;(2)Tiny lymph nodes:456tiny lymph nodes were found in study group,with an average of(8.1±1.2)per case and in control group there were 218,with an average of(4.2±2.3)per case;(3)Total number of lymph nodes:1847lymph nodes were detected in study group,with an average of(26.5±3.2)per case,and in control group there were 1289,with an average of(15.2±5.3)per case.(4)The incidence of postoperative sensory disability:the incidence of postoperative sensory disability was 12.5%(7/56)in study group and 67.8%(38/56)in control group.And the differences were statistically significant(P〈0.05).Conclusion The lymph tracing with carbon nanoparticles suspension injection can increase the seized rate of lymph nodes.The intercostobachial nerve can be preferably retained by negative development.
