摘要
目的:评价经皮椎间孔镜技术(PTED)治疗腰椎间盘突出症(LDH)的近期疗效。方法:选择2015年2月至2016年6月行经皮椎间孔镜下腰椎间盘摘除术的单节段腰椎间盘突出症患者30例,采用疼痛视觉模拟评分(VAS)评价患者腰腿痛缓解情况,改良Macnab疗效评定标准评估临床疗效。结果:所有病例均顺利完成手术,手术时间60~120 min,出血30~60 mL,术后1 d、1周及术后3月VAS评分明显低于术前,差异有统计学意义,P<0.01。术后3月以改良Macnab标准评价临床疗效:优23例,良4例,可3例,优良率达90%,所有患者无一例发生硬脊膜破裂、神经根损伤、感染等并发症,术后复查MRI显示突出椎间盘基本消失。结论:经皮椎间孔镜下腰椎间盘摘除术治疗腰椎间盘突出症安全有效,具有创伤小、出血少、术后恢复快、住院时间短等优势。
Objective:To evaluate the short-term efficacy of percutaneous transforaminal endoscopic discectomy in the treatment oflumbar disk herniation.Methods:This study selected randomly30patients with single-segment lumbar disk herniation on whompercutaneous transforaminal endoscopic nucleus pulposus discectomy was conducted from February2015to June2016.The clinicalefficacy of treatment was evaluated by using the pain visual analog scale(VAS score)and the criteria of the improved Macnab.Results:All the surgeries on the30patients were successfully completed with the operation time of60-120min and blood loss of30-60mL.The postoperative VAS score within one day,one week and three months after surgery was significantly lower than thepreoperative one.The comparative result(P<0.01)showed significantly statistical differences.According to the criteria of theimproved Macnab evaluation,the excellence rate was much higher with excellent in23cases and good in4cases,and the rate ofexcellent and good was90%.There were no complications such as dural rupture,nerve root injury,infection and other complications inall patients,and MRI examination showed that the protrusion of intervertebral discs basically disappeared.Conclusion:Percutaneouslumbar disc herniation is a safe and effective way to treat the patients with lumbar disc herniation with less trauma,less blood loss,faster rehabilitation,shorter postoperative hospital stay and other advantages alike.
