摘要
目的对比原发性肝癌三种亚型的外科诊断治疗效果,讨论混合型肝癌的临床病理特点。方法选择15例混合型肝癌患者、120例肝细胞癌患者、35例胆管细胞癌患者的临床病理资料进行对比讨论分析,主要考察血液学(肝炎病毒的感染情况、血清甲胎蛋白含量)、诊疗手段(手术及其他治疗)、病理学资料(合并肝硬化、血管侵犯、淋巴结转移)三个方面。结果有两例患者在术前接受诊断,其余13例混合型肝癌患者在术后进行了病理及免疫组织化学诊断。有8例根治性切除、7例姑息性切除,结果与肝细胞癌与胆管细胞癌类似。混合型肝癌的病例特点为:合并门静脉癌栓子和淋巴结转移情况较肝细胞癌常见,同胆细胞癌类似;乙肝、丙肝病毒的感染情况、合并肝硬化、血清甲胎蛋白等指标与肝细胞癌类似,但和胆管细胞癌差异大。结论混合型肝癌术前确诊难、恶性的程度高、病程发展快。
Objective To compare the clinical diagnosis and treatment of three subtypes of primary liver cancer,and to discuss the clinicopathological features of mixed liver cancer.Methods The clinical and pathological data of15patients with hepatocellular carcinoma,120patients with hepatocellular carcinoma and35patients with cholangiocarcinoma were selected from March2000to March2006,and the hematology(hepatitis virus Infection,serum alpha-fetoprotein content),diagnosis and treatment(surgery and other treatment),pathological data(combined with cirrhosis,vascular invasion,lymph node metastasis)three aspects.Results Two patients were diagnosed preoperatively.The remaining13patients with mixed liver cancer underwent pathological and immunohistochemical diagnosis.There are8cases of radical resection,7cases of palliative resection,the results and hepatocellular carcinoma and cholangiocarcinoma similar.Mixed liver cancer cases are characterized by:portal vein tumor emboli and lymph node metastasis than liver cell carcinoma common,similar to the same as cholangiocarcinoma;hepatitis B,hepatitis C virus infection,liver cirrhosis,serum alpha-fetoprotein and other indicators and liver cells Cancer is similar,but different from cholangiocarcinoma.Conclusion The diagnosis of mixed liver cancer is difficult,the degree of malignancy is high and the course of disease is developing rapidly.
