摘要
目的探究客观听功能检测对于听力正常的主观性耳鸣患者的临床意义。方法按标准选取2014年1月~2016年12月在我院耳鼻喉科就诊的主观性耳鸣患者60例(107耳)作为试验组,另外选取60名(120耳)健康无耳鸣者作为对照组。所有患者均行耳声发射检测(OAE),具体包括TEOAE和DPOAE,以及声导抗及听性脑干电位,数据进行统计学分析。结果对照组TEOAE和DPOAE检出率均高于试验组,差异具有统计学意义(P<0.05);当频率为3~8k Hz时,对照组DPOAE频率幅值均高于试验组,差异具有统计学意义(P<0.05);两组各频带的TEOAE幅值平均值,差异具有统计学意义(P<0.05);与对照组相比,试验组Ⅰ波潜伏期明显延长,差异有统计学意义(P<0.05)。试验组Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波间期与对照组比较,差异无统计学意义(P>0.05)。结论主观性耳鸣患者主要病变位于耳蜗,且OAE检测可以尽早地反应外毛细胞的功能状态,为患者的诊断和鉴别提供有力依据,而DPOAE在高频段更有优势。
Objective To explore the clinical significance of objective auditory function test in patients withsubjective tinnitus with normal hearing.Methods Sixty patients(107ears)of subjective tinnitus who were treated withENT in our hospital from January2014to December2016were selected as the experimental group.60patients(120ears)with no tinnitus were selected as the control group The All patients were treated with otoacoustic emissionsafter admission,including TEOAE and DPOAE,as well as acoustic impedance and auditory brainstem potential,thedata were analyzed statistically.Results The detection rate of TEOAE and DPOAE in the control group was higherthan that in the experimental group(P<0.05).When the frequency was3～8kHz,the amplitude of DPOAE in thecontrol group was higher than that in the experimental group,the difference was statistically(P<0.05).Comparedwith the control group,the latency of the groupⅠwas significantly longer than that of the control group(P<0.05)).The difference was statistically significant(P<0.05).There was no significant difference between the experimental groupⅠ～Ⅲ,Ⅲ～Ⅴ,Ⅰ～Ⅴand the control group(P>0.05).Conclusion The main lesion of the subjective patients islocated in the cochlea,and the OAE test can respond to the functional status of the outer hair cells as soon as possible.Itprovides a strong basis for the diagnosis and identification of the patients,while DPOAE is more advantageous in the highfrequency band,and the dominant band of TEOAE needs us In the future to further study to confirm
