摘要
目的:对全科团队模式在上海市高桥社区糖尿病管理的作用与效果进行评价分析。方法:通过对上海市高桥社区糖尿病患者开展全科团队模式管理,比较实施前后糖尿病管理指标、糖尿病患者生化指标及糖尿病知识知晓率的变化。结果:全科团队模式管理实施后患者的管理率、登记率、随访率、规范管理率、糖尿病知识知晓率明显提高(P<0.01);患者的空腹血糖、餐后2 h血糖、糖化血红蛋白、总胆固醇明显下降(P<0.05);服药依从率明显提高(P<0.05)。结论:全科团队模式可有效提高社区糖尿病患者管理效果,值得开展。
Objective:To analyze and evaluate the role and effect of general practitioner team mode in the management of diabetesin Shanghai Gaoqiao community.Methods:Using general practitioner team management for diabetes patients in Shanghai Gaoqiaocommunity,compared the changes of diabetes management index,biochemical index and knowledge of diabetes mellitus beforeand after the implementation.Results:The management rate,registration rate,follow-up rate,standardized management rate andawareness rate of diabetes knowledge were significantly improved after the implementation of general team management(P<0.01).The fasting blood glucose,postprandial2h blood glucose,glycosylated hemoglobin,total cholesterol decreased significantly(P<0.05),and the medication compliance rate increased significantly(P<0.05).Conclusion:General team model can effectivelyimprove the effectiveness of community diabetes management.It is worth developing.
