摘要
目的探讨完全腹腔镜下胰十二指肠切除术的可行性及手术要点。方法回顾性分析1例完全腹腔镜胰十二指肠切除术患者的临床资料,包括术前诊断、术中难点及对策、术后恢复情况等。结果手术顺利,无术中严重并发症,手术时间8.5h,术中出血约500mL;患者术后恢复快、无严重并发症,术后住院10d。结论娴熟的腹腔镜下操作技术是完成胰十二指肠切除术的先决条件;掌握好手术适应证,选择性地开展腹腔镜下胰十二指肠切除术是安全、可行。
Objective To investigate the feasibility and key points of total laparoscopic pancreaticoduodenectomy.Methods Clinical data of one patient who underwent total laparoscopic pancreaticoduodenectomy were analyzed retrospectively, including preoperative diagnosis, intraoperative difficulties and countermeasures, and postoperative recovery. Results The operation was completed successfully. The operation time, intraoperative blood loss and hospital stay were 8. 5 hours, 500 mL and 10 days, respectively. Patients achieved quick postoperative recovery and had no severe complications. Conclusion Skilled laparoscopic technique is a prerequisite for the completion of pancreaticoduodenectomy. It is safe and feasible to selectively perform laparoscopic pancreaticoduodenectomy under the condition of grasping the indications for operation.
