PCR熔解曲线法在原发性高血压患者中MTHFR677T基因多态性检测中的应用

摘要 目的:探讨PCR熔解曲线法检测亚甲基四氢叶酸还原酶(MTHFR)基因C677T多态位点在原发性高血压(EH)诊治中的准确性,并分析血浆Hcy的水平、H型高血压发生率与MTHFR 677C/T基因型的相关性。方法:收治EH患者300例,均采用PCR熔解曲线法进行MTHFR 677C/T基因型检测,并与DNA测序法对比。结果:①与DNA测序法比较,差异无统计学意义(P>0.05)。②MTHFR C677T基因的TT基因型的Hcy水平显著高于CC+CT基因型的Hcy水平,男女差异有统计学意义(P<0.05)。③MTHFR C677T基因的TT基因型较CC+CT基因型H型高血压发病风险显著增加,男女差异有统计学意义(P<0.05)。结论:PCR溶解曲线法不但操作简单,而且准确率高,进行高血压患者的MTHFR C677T基因型检测有深远的临床意义,可早期预防H型高血压。 Objective:To explore the accuracy of PCR melting curve method in detection of polymorphism of MTHFR C677T gene in patients with primary hypertension,and analyze the correlation between plasma Hcy level,H type hypertension incidence rate and MTHFR 677C/T genotype.Methods:300 patients with EH were selected.They were given MTHFR 677C/T genotype detection by PCR melting curve method,and the results were compared with the DNA sequencing method.Results:①Compared with DNA sequencing method,the difference was not statistically significant(P>0.05).②The level of Hcy of TT genotype in MTHFR C677T gene was significantly higher than CC+CT genotype;male and female differences were statistically significant(P<0.05).③The risk of H type hypertension of TT genotype in MTHFR C677T gene was higher than CC + CT genotype;male and female differences were statistically significant(P<0.05).Conclusion:PCR melting curve method was simple,and the accuracy was high.It had profound clinical significance for the detection of MTHFR C677T genotype in patients with hypertension,which can prevent the early H type hypertension.
