摘要
目的分析乳腺癌患者PICC置管非计划性拔管的原因,并探讨预防措施。方法对我科在2010年3月至2013年3月间经外周留置中心静脉导管(PICC)的403例乳腺癌患者中非计划拔管的96例患者进行回顾性分析。结果患者要求拔管、管道堵塞、导管脱出、血栓形成等是造成非计划性拔管的主要原因,与患者自身因素、医护人员因素、置管时间、管道材质等有关。结论做好置管前充分评估,选择合适的PICC管道,保证各项护理操作的规范化,加强带管期间的维护和观察,可明显降低非计划性拔管发生率。
Objective To analyze the reasons of unplanned extubation(UEX)of PICC catheter in patients with breast cancer and explore the preventive measures.Methods During the March 2010 to March 2013,we analyzed retrospectively 96 unplanned extubation patients of 403 cases of patients with breast cancer which were in peripherally inserted central catheter(PICC).Results Patient's demand,catheter jam,emergence of catheter,thrombosis etc is the main cause of unplanned extubation.It is connected with the patient's own factors,medical staffs,catheter time,catheter material and so on.Conclusion Making full assessment before catheterization,choosing the appropriate PICC catheter,ensuring the standardization of care operations,strengthening the maintenance and observation in the period of indwelling catheter can obviously reduce the incidence of unplanned extubation.
