Effects of Recombined Human Growth Hormone(rh-GH)on Endotoxemia of Severe Intraabdominal Infection Induced by Escherichia Coli in Rats

摘要 目的 :观察重组人生长激素 (rh -GH)对严重腹腔内大肠杆菌感染大鼠内毒素血症的影响。方法 :SD大鼠 75只 ,随机分为正常对照组、感染组和治疗组 (后两组又再分为 1天、 3天两个亚组 ) ,每组 15只。感染组 ;腹腔注射鸵鸟株大肠杆菌标准菌株悬液 (1× 10 10 cfu/L ,15ml/kg) ;治疗组 :腹腔注射大肠杆菌后给予rh -GH (2 2 5iu .kg-1.d-1)肌肉注射治疗。于注射后 2 4h、 72h分别测定各组大鼠的血浆内毒素含量、血浆TNFα浓度、白细胞总数和分类计数及平均动脉压 (MAP)。结果 :感染组血浆内毒素含量显著升高 ,2 4h和 72h分别为(0 2 5 6± 0 0 5 2 )和 (0 189± 0 0 5 2 )EU/ml,明显高于对照组 [(0 111± 0 0 5 3)EU/ml,P <0 0 1],血TNFα明显升高 [(3 5 9± 0 6 9) /(2 88± 0 74) μg/L ,P <0 0 5 ],呈现典型而严重的感染性休克临床经过 ,MAP显著降低[ (6 7 4± 2 2 6 ) / (12 4 6± 13 9)mmHg,P <0 0 1];治疗组血浆内毒素含量在感染后 2 4h轻度升高 [(0 15 0±0 0 39)EU/ml],72h降至正常对照水平 [(0 10 8± 0 0 42 )EU/ml],与感染组比较 ,两个时点均有显著差异 (P <0 0 1) ,72h血TNFα浓度 [(1 5 4± 0 36 ) μg/L]明显低于对照组 [(2 88± 0 74) μg/L ,P <0 0 1]和感染组[(2 Objective:To investigate the effects of recombined human growth hormone(rh-GH) on endotoxemia of severe intraabdominal infection induced by Escherichia coli(E.coli)in rats.Methods:Seventy-five rats were randomly divided into 5 groups(n=15):normal control group,2 infection groups(1-day group and 3-day group),and 2 treatment groups.Introabdominal injection of E.coli(1×10 10 cfu/L,15ml/kg)was given to all the animals of infection groups and treatment groups,at the same time rh-GH(2.25 iu.kg -1 .d -1 )i.m.was given to the treatment groups.Measured the levels of plasma endotoxin and TNFα,mean arterial blood pressure(MAP),and peripheral WBC counting at 24 hours as well as 72 hours after the infection.Results:In the infection groups,plasma endotoxin levels at 24h[(0.256±0.052)EU/ml]and 72h[(0.189±0.052)EU/ml]were significantly higher than that of normal control group[(0.111±0.053)EU/ml,P<0.01];plasma TNFα level was increased remarkably[(3.59±0.69)/(2.88±0.74)μg/L,P<0.05]at 24 hours after infection;typical clinical development of septic shock occurred;MAP was significantly reduced[(67.4±22.6)/(124.6±13.9)mmHg,P<0.01];While in the treatment groups,plasma endotoxin levels were(0.150±0.039)EU/ml at 24h and(0.108±0.042)EU/ml at 72h,both were significantly lower than those of infection groups(P<0.01);plasma TNFα level at 72h[(1.54±0.36)μg/L]was significantly lower than that of control grop(2.88±0.74)μg/L,P<0.01]and infection groupand infection group[(2.23±1.09)μg/L,P<0.05];MAP was relatively stable[(114.4±15.9)/(124.6±13.9)mmHg];peripheral WBC and neutrophil at 24h were higher than those of infection group.Conclusions:rh-GH may contribute to reduce the plasma levels of endotoxin and TNFα in severe intraabdominal infection rats induced by E.coli,ameliorate the clinical manifestation of septic shock and endotoxemia,help to locate the infection from proliferation and maintain the normal blood pressure,further improve the animals' outcomes.
