摘要
目的 :探讨功能性鼻窦内窥镜术 (FunctionalEndoscopicSinusSurgery,FESS)治疗毛霉菌性非侵袭型上颌窦炎 (mucoraceaenon -invasivemaxillarysinusitis,MNIMS)及相关因素。方法 :1997年～ 2 0 0 1年我科对 44例真毛霉菌性非侵袭型上颌窦炎行FESS治疗。本研究主要对手术径路及抗真菌药物氟康唑的使用对疗效及转归的影响进行了分析。结果 :44例在术后不同时间内均获治愈。复合术式 (经上颌窦前壁开孔加上颌窦自然开口 )的术后愈合期 3 81± 2 11周 ,而经上颌窦自然开口术式的术后愈合期为 8 5 7± 3 45周 ,两组有显著性差异 (P <0 0 1)。两种术式术后各分为两组 ,分别使用氟康唑和 /或 0 9%生理盐水冲洗术腔 ,氟康唑Ⅰ组愈合所需时间为8 40± 3 76周 ;对照Ⅰ组愈合所需时间为 8 33± 3 88周。氟康唑Ⅱ组所需时间为 4 32± 2 77周 ;对照Ⅱ组愈合所需时间为 4 2 9± 2 98周。将结果行统计学处理 ,无显著性差异 (t检验 ,P >0 0 5 )。结论 :FESS是治疗MNIMS的有效方法 ;经上颌窦前壁开孔加上颌窦自然窦口复合术式的疗效比单一经上颌窦自然窦口术式好 ;用抗真菌药物氟康唑冲洗术腔对疗效无影响 ;术后坚持在鼻窦内窥镜下换药 ;换药时间不少于
Objective:To explore the treatment of mucoraceae non-invasive maxillary sinusitis (MNIMS) by functional endoscopic sinus surgery(FESS)and the related factors influencing the result.Methods:Forty-four cases of MNIMS that received FESS were studied,and the operative approache and application of fluconazole (FCZ)that influenced both result and recovery was analyzied.Result:The period of convalescene by the combined approaches(via ostium of maxillary sinus and canine fossa)were 3.81±2.21 wks,and from ostium of maxillary sinus only 8.57±3.45 wks. There was signicant difference between the two groups (P<0.01).in respect to the period of convalescene.The period of convalescence of the group using FCZ to douche the cavities was 8.33±3.88 wks,and the group without using FCZ was 8.40±3.76 wks.There were no statistics signicance(P>0.05).The period of convalescnce of the group using FCZ to douche the cavities were 4.29±2.98 wks and the group without using FCZ were 4.32±2.77 wks.There were no statistics signicance(P>0.05).Conclusion:FESS was an effective approach for treatment.The operation of FCZ approach was superior to that of via ostium of maxillary sinus only.There were no signicance from douching the operative cavities with FCZ or not(P>0.05).
