摘要
目的 探讨经鼻内镜中下鼻道联合开窗术治疗真菌性上颌窦炎的临床疗效.方法 总结本科2011 年3 月-2015 年9 月收治的57 例真菌性上颌窦炎的临床资料,分析其临床表现、CT 表现、手术方法及疗效.结果 全部病例术后6 个月进行评价,经鼻内镜中鼻道上颌窦造口术治疗真菌性上颌窦炎37 例,其中复发4 例,迁延性炎症8 例,窦口狭窄3 例,治愈率（89.19%）.经鼻内镜中下鼻道联合开窗术20例,复发1 例,开窗口闭锁1 例,治愈率（95%）,两组有统计学差异.结论 经鼻内镜中下鼻道联合开窗术是治疗真菌性上颌窦炎的一种方便有效的手术途径,并发症少,治愈率高,值得在基层医院推广.
objective To investigate the therapeutic effect of the middle and inferior meatus combined fenestration operation under nasalendoscopic in treatment of fungal maxillary sinusitis （FMS）. Methods From March 2011 to September 2015, clinical of data of 57 with FMS in theoffice were summarized, and analysed the clinical manifestation, CT characteristic, surgical method and effect. Results Of all the cases were estimated6 months after the surgery, 37 cases with FMS were treated by the middle meatus maxillary fenestration operation under nasal endoscopic, 4 casesshowed recurrence, 8 cases showed protracted inflammation, 3 cases showed sinus stenosis, the cure rate （89.19%） among them. However, 20 cases weretreated by middle and inferior meatus combined fenestration operation under nasal endoscopic, 2 cases showed recurrence and windowing atresiarespectively, the cure rate （95%） among them,which had significant difference between two groups. Coclusions Middle and inferior meatus combinedfenestration operation under nasal endoscopic is conwenient and effective operation, few complication, high cure rate. It is worth popularizing inbasic-level hospitals.
