声学造影定量评价急性肾功能衰竭肾皮质血流灌注的实验研究 被引量:3

Quantitative Assessment of Renal Cortex Blood Perfusion of Acute Renal Failure by Intravenous Contrast Ultrasound: an Experimental Study

摘要 目的 探讨应用自制白蛋白氟碳声学造影剂和声学密度定量技术定量评价急性肾功能衰竭 (ARF)肾皮质血流灌注的可行性。方法 兔后腿肌注 5 0 %甘油 (12~ 15ml/kg) ,建立ARF动物模型。分别于注射甘油前和注射甘油后第三天应用声学造影剂和声学密度定量 (AD)技术检查肾脏 ,观察造影增强效果 ,测定肾皮质造影前后峰值密度 (PI)、曲线下面积 (AUC)和平均通过时间 (MTT)。结果 正常肾脏造影增强效果明显好于ARF肾 ,二者视觉判断差异显著 ;正常肾和ARF肾造影后肾皮质PI、AUC均比造影前明显增加 ,但ARF肾皮质造影后PI、AUC却低于正常肾皮质造影后PI、AUC(P<0 .0 1) ,MTT则变化不大。结论 急性肾小管坏死性肾衰竭时 ,由于肾皮质血流灌注减少 ,引起造影后PI、AUC等有关反映血流灌注的参数也随之下降 ,与正常肾脏差异显著。因此 ,采用经静脉声学造影和声学密度定量技术 ,能够对ARF时肾皮质血流灌注作出定量评价。 Objective To explore the feasibility of quantitative evaluation for renal cortex blood perfusion of acute renal failure (ARF) in rabbits using self made albumin perfluorocarbon ultrasound contrast agent and acoustic densitometry(AD) technique. Methods The model of ARF was established by the intramuscular injection of 50% glycerol (12~15ml/kg) into the hind leg. Ultrasonic examination was performed before and on the 3rd day after the injection of glycerol by using intravenous contrast ultrasound and AD technique. Some parameters related to the time intensity curve such as peak intensity (PI), the area under the curve (AUC) and the mean transit time (MTT) of renal cortex were measured before and after intravenous administration of contrast agent. Results The imagings of renal cortex of normal kidneys were obviously enhanced than those of ARF kidneys. Furthermore, in all of the kidneys, the PI, AUC of renal cortex after intravenous administration contrast agent significantly increased compared to those of before administration contrast agent ( P <0.01). But they were much lower in the ARF kidneys than those in the normal kidneys, and the great change was not observed in MTT before and after ARF. Conclusion During ARF, the PI, AUC of renal cortex after intravenous administration of contrast agent significantly decreased as a result of reduction of renal blood perfusion. So, intravenous contrast ultrasound and acoustic densitometry can be used to quantitatively assess renal blood perfusion in ARF.
