摘要
目的观察大量输血对外伤性失血患者的凝血功能、炎症因子水平的影响。方法针对2014年12月-2015年12月选择于我院接受治疗的外伤性失血患者45例进行分析,所有患者均大量输血治疗;对比分析所有患者于输血前后的血小板计数(BPC)变化、凝血功能各项指标[PT(血浆凝血酶原时间)、Fbg(纤维蛋白原)、TT(血浆凝血酶时间)、a PTT(活化部分凝血活酶时间)]变化以及炎症因子水平(TNF-α、IL-8、IL-10)的变化。结果在输血前后的相互对比中,所有患者输血后的BPC增加; PT、TT、a PTT上调,但FIbg下调;炎症因子:TNF-α、IL-8、IL-10均较其输血前升高,所有观察指标的比较,差异均具有统计学意义(P<0.05)。结论对外伤性失血患者而言,予以大量输血的措施虽然在一定程度上可成为其抢救生命的重要举措,但是会影响患者的正常凝血功能以及炎症因子水平,因而在实施大量输血时,要严格监测患者的凝血功能、炎症因子水平,以防止并发症发生。
Objective To observe and analysis of massive blood transfusion in traumatic blood coagulation function in patients,the significance of the level of inflammatory factors.Methods for the December 2014-2015 year in December trauma treated in our hospital 45 cases of patients with blood loss were analyzed,all patients were given a full range of observation and the amount of blood transfusion;comparative of blood platelet count（BPC） changes in all patients before and after blood transfusion,blood coagulation index PT（prothrombin time） 、Fbg（fibrinogen）,TT（thrombin time） 、aPTT,（activated partial thromboplastin time）changes And the levels of inflammatory cytokines（TNF-,IL-8,IL-10）.Results Compared with each other before and after blood transfusion,the BPC of all patients increased after blood transfusion,but FIbg was downregulated,and the inflammatory factor：TNF-α IL-8 IL-10 was higher than that before and after transfusion,and the difference of all the observed indexes was statistically significant（P〈0.05）.Conclusion For the patients with traumatic blood loss,the measure of massive blood transfusion may be an important measure to save their lives to some extent,but it will affect the normal coagulation function and the level of inflammatory factors of the patients.Therefore,in carrying out a large number of blood transfusions,the coagulation function and inflammation of the patients should be strictly monitored.Factor level to prevent complications.
