摘要
[目的]分析宁夏某三级公立医院2009-2016年医疗收入结构变化情况及各医疗服务项目对医疗收入结构的影响,为优化该院医疗收入结构提供参考和决策依据。[方法]采用结构变动度的方法,分析该院2009-2016年医疗收入结构的变化情况。[结果] 2009-2016年该院门诊收入结构变动度为41. 112%,平均每年的结构变动度为5. 139%,检查收入对门诊收入结构变动贡献率最大,为33. 576%;住院收入结构变动度为84. 793%,平均每年结构变动度为10. 599%,药品对住院收入结构贡献率最大为40. 284%。[结论]检查收入是影响该院门诊收入变动的主要因素,药品收入是影响该院住院收入变动的主要因素。该院应建立完善的医院费用核查系统,促进检查和药品收入合理化。同时,不断优化医疗服务项目收费结构,提高医院管理水平,从根本上保障医院的经济效益和社会效益。
Objective To analyzes medical income structure change in a tertiary public hospital from 2009 to 2016 in Ningxia and the influence of medical services to medical income structure,so as provide reference and decision basis for optimizing income structure of hospital medical income. Methods The study evaluated medical income structure change based on structural variation from 2009 to 2016. Results From 2009 to 2016,the structural variation degree of outpatient income was 5.139%,examination income had the biggest contribution rate to outpatient income（33.57%）,the structural variation degree of hospitalization was 84.793%. The structural variation degree of annually average was 10.599%,drug income had the biggest contribution rate to hospitalization income（40.284%）. Conclusions Examination income is main influence factor of influencing outpatient income in this hospital,and drug income is main influence factor of influencing hospitalization income. That hospital should build up perfect hospital cost inspect system,promote examination and drug income rationalization. At the same time,we constantly optimize the charging structure of medical service projects,improving the management level of hospitals,and fundamentally guarantee the economic and social benefits of hospitals.
