妊娠期糖尿病孕妇及新生儿白细胞端粒长度改变及其影响因素分析 被引量:2

Changes of leukocyte telomere length of pregnant women with gestational diabetes mellitus and their newborns and its related influential factors

摘要 目的分析白细胞端粒长度(LTL)分别在妊娠期糖尿病(GDM)孕妇孕晚期外周血及新生儿脐血中的改变及其影响因素。方法选取2010年10月-2011年4月于该院产科分娩的孕妇及新生儿为研究对象,共纳入数据资料完整且标本采集齐全的26对GDM母子对作为GDM组。另随机选取47对75 g口服糖耐量试验(OGTT)阴性的孕妇及其新生儿为对照组。检测孕妇孕晚期外周血和新生儿脐血血浆瘦素、8-异前列腺素F2α(8-iso-PGF2α)、生化及糖脂代谢指标水平,RT-PCR检测白细胞相对端粒长度,多元线性回归分析端粒长度改变的影响因素。结果 GDM组孕妇孕晚期血LTL较对照组明显缩短(P<0. 05),而两组间新生儿脐血LTL比较,差异无统计学意义(P>0. 05),但对照组女婴脐血LTL相对较男婴长(P<0. 05)。经多元回归分析发现,家族糖尿病史、孕晚期血高8-iso-PGF2α水平和高瘦素水平与GDM孕妇孕晚期LTL缩短密切相关(标准偏回归系数分别为-0. 279、-0. 292及-0. 254,均P<0. 05)。结论 GDM孕妇孕晚期外周血LTL明显缩短,糖尿病家族史、高氧化应激水平及高瘦素水平均可能是引起其LTL加速缩短的重要危险因素。 Objective To investigate the changes of leukocyte telomere length (LTL) in both of the peripheral blood of pregnant women with gestational diabetes mellitus (GDM) and the cord blood of their newborns, and the associated influencing factor. Methods Pregnant women and their newborns who gave birth in the hospital from October 2010 to April 2011 were selected as the research ohjects. A total of 26 pairs of mother-to-child pairs of GDM with complete data and complete sample collection were included as the GDM group. In addition, 47 pairs of pregnant women with a negative oral glucose tolerance test (OGTT) of 75 g and their newborns were randomly selected as the control group. The plasma level of leptin, 8-iso-PGF2α and biochemical and glyeolipid metabolism indexes in both of the peripheral blood and cord blood were detected, The relative LTL was measured through tiT-PCR. Finally, the influencing factor for the changes of LTL was analyzed by muhiple linear regression. Results The LTL in the third trimester peripheral blood of pregnant women with GDM was significantly shortened than conlrols ( P〈 0. 05 ) , whereas no significance was found in the cord blood of their newi-orns between two groups ( P 〉 0. 05 ) . Only the LTL of female newborns was found relatively longer than the male newborns in the control group ( P〈 0. 05 ) . Finally, the multiple linear regression revealed that the family history of diabetes, increased plasma level of 8-iso-PGF2α and lep- tin in peripheral blood were significantly associated with the shortened LTL of pregnant women with GDM, the standard partial regression cof- ficents were -0. 279, -0. 292, and -0. 254, all P〈0. 05) . Conclusion The pregnant women with GDM have shorter LTL in the third tri- mester, and it might be attrihuted to the family history of diabetes, higher plasma level of oxidative stress and leptin in the peripheral blood.
