摘要
目的观察DSA诊断及介入治疗对缺血性脑卒中患者颈部血管狭窄的疗效及生存质量。方法选择新疆医科大学附属中医医院和云南中医学院第一附属医院2014年5月至2016年7月收治的缺血性脑卒中并行DSA的患者共103例为研究对象,78例患者进行介入手术,观察不同检查对颈部血管的诊断情况,介入术后的即刻疗效、术后随访结果及并发症情况。对患者术前、术后1周相关神经功能进行比较。结果 DSA较CTA和MRA,能检出更多病例及狭窄在70%~99%、50%~69%及<50%的血管。78例共88支症状性动脉粥样硬化重度狭窄的血管进行介入手术,手术成功率100%,术后78例患者88支血管的狭窄率明显减少,1周后神经功能症状明显减轻,经随访3月~2 a,介入手术的血管无再次狭窄情况发生,主要终点事件发生率为3.85%。结论 DSA诊断及介入治疗对缺血性脑卒中患者颈部血管狭窄具有较好疗效,可明显改善患者神经功能症状。
Objective To observe the efficacy and quality of life of DSA diagnosis and interventional treatment of cervical stenosis in patients with ischemic stroke. Methods A total of 103 patients with ischemic stroke admitted to our hospital from May 2014 to July 2016 were enrolled in this study. Interventional procedures were performed in 78 patients. Diagnosis,interventional immediate effect,postoperative follow-up results and complications were observed. Patients before surgery,one week after surgery-related neurological function were compared. Results All 103 patients underwent CTA,MRA and DSA. Compared with CTA and MRA,DSA could detect more cases and blood vessels with stenosis of 70%-99%,50%-69% and 〈50%. The success rate of78 cases out of 88 symptomatic atherosclerosis severe vascular interventional surgery was 100%.The stenosis of vascular significantly reduced, one week after the interventional surgery and their neurological symptoms were significantly reduced. At follow-up 3 months-2 years,there was no restenosis in the interventional vessel,and the incidence of the primary endpoint was 3.85%. Conclusion DSA diagnosis and interventional treatment of ischemic stroke patients with cervical stenosis has a good effect and can significantly alleviate the patient's neurological symptoms.
