Application of Intravascular Ultrasound in the Diagnosis and Interventional Treatment of Coronary Artery Critical Lesions

摘要 为探讨血管内超声在冠状动脉临界病变的诊断及介入治疗中的应用价值,本研究选取我院2015年8月至2016年7月收治的60例经冠状动脉造影提示为冠状动脉临界病变患者为研究对象,所有患者均接受血管内超声检查,根据血管内超声检测的最小管腔面积≤4.0mm^2作为介入治疗的标准,分为治疗组(n=28)和对照组(n=32)。治疗组采用置入冠脉支架治疗,对照组仅采用药物强化治疗;比较两组冠状动脉病变的性状及心脏事件发生率。结果显示,血管内超声对于偏心斑块、钙化斑块的检出率均明显高于冠状动脉造影,差异具有统计学意义(p〈0.05);治疗组软斑块、偏心性检出率明显高于对照组,而心脏事件发生率明显低于对照组,差异具有统计学意义(p〈0.05);两组钙化及硬斑块检出率比较,差异无统计学意义(p〉0.05)。本研究认为,对冠状动脉临界病变患者采用血管内超声检查,可明确临界病变的狭窄程度及性质,对于介入治疗的开展具有一定的指导价值。 In order to investigate the value of intravascular ultrasound in the diagnosis and interventional treatment of coronary artery critical lesions, 60 patients in our hospital with coronary artery angiography, who underwent coronary angiography from August 2015 to July 2016, were selected as patients with coronary artery disease. All patients underwent endoscopic ultrasonography. According to the minimum lumen area of intravascular ultrasound 〈4.0 mm^2 as the standard ofinterventional therapy, all patients were divided into treatment group (n=28) and control group (n=32). The treatment group was treated with coronary stenting, and the control group was only treated with drug intensive therapy. We compared the traits of coronary lesions and the incidence of cardiac events in the two groups. The results showed that the detection rate of eccentric plaque and calcified plaque was significantly higher than that of coronary angiography. The difference was statistically significant (p〈0.05). The detection rote of soft plaque and eccentricity was significantly higher in the treatment group. In the control group, the incidence of cardiac events was significantly lower than that of t he control group, and the difference was statistically significant (p〈0.05). There was no significant difference in the detection rate of calcification and hard plaque between the two groups (p〉0.05). This study believed that the use of intmvascular ultrasound in patients with coronary artery disease could be used to determine the extent and nature of critical lesions, and it has a certain guiding value for the development of interventional therapy.
