飞秒激光小切口角膜基质透镜取出术透镜残留的处理

Management of remnant intrastromal lenticule of small incision lenticule extraction

摘要 目的分析飞秒激光小切口角膜基质透镜取出术(SMILE)透镜残留的处理及其效果。方法对2015年4月至2017年3月行SMILE术864例(1715眼)的临床资料进行回顾性研究,根据是否发生基质透镜残留分为透镜残留组和正常手术组,分析透镜残留其原因、临床表现、处理及其效果。结果SMILE术中透镜残留的发生率为0.29%,屈光度低及透镜分离困难是主要原因。残留透镜取出后效果良好,术后1个月视力和屈光度与正常手术组差异无统计学意义。Pentacam检查显示透镜残留眼角膜周边部分区域陡峭伴前表面高度增加,相邻内侧区域曲率异常降低。结论透镜残留是SMILE手术的少见并发症,及时取出结果良好。 Objective To analyze the management and efficacy of remnant intrastromal lenticule during and after femtosecond laser-assisted small incision lenticule extraction (SMILE). Methods The data of 1 715 eyes of 864 cases with SMILE between Apr. 2015 and Mar. 2017 were retrospectively analyzed. All cases were divided into remnant group and normal group based on remnant intrastromal lenticule occured. The cause, clinical characteristics, treatment and efficacy were analyzed. Results The rate of remnant intrastromal lenticule was 0.29%. Lower diopter and difficult lenticule extraction were the main risk factors. The difference of visual acuity and refractive outcoms at 1 month after surgey were not statistically significant between two groups. Corneal topography demonstrated a peripheral area of steepened corneal surface, and front elevation was relatively higher in this area. Abnormal lower curvature was detected in the adjacent area. Conclusion Remnant intrastromal lenticule is a rare complication, good efficacy was achieved with appropriate management.
