摘要
目的:探讨血清胃蛋白酶原(PG)、胃泌素-17(G-17)及幽门螺杆菌(Hp)在筛查胃癌和癌前疾病中的临床价值。方法:选取某院因上消化道不适收治的178例患者为观察组,选取同时段健康体检者50例为对照组,所有患者均采用酶联免疫吸附试验(ELISA)检测胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、胃泌素-17,14C呼气试验检测Hp感染,对比分析其在胃癌及其癌前疾病中的差异。结果:(1)与对照组相比,萎缩性胃炎组、早期胃癌组及进展期胃癌组PGⅠ和PGR水平均明显下降,PGⅡ水平明显升高(P <0. 05);胃溃疡组PGⅠ,PGⅡ明显升高,PGR水平降低(P <0. 05)。(2)与对照组相比,胃息肉组、胃溃疡组、早期胃癌组、进展期胃癌组G-17水平均明显升高(P <0. 05)。(3)各组间Hp感染率差异无统计学意义(P> 0. 05);早期胃癌组、进展期胃癌组的Hp阳性患者胃泌素分泌明显增加(P <0. 05)。结论:血清PG和G-17可作为胃癌及癌前疾病诊断的重要指标,在胃癌患者中,推测Hp感染后可通过某种途径影响G-17的合成和释放。
Objective :To explore the level of serum pepsinogen ( PG), Gastrin-17 ( G-17 ) and clinical value of helicobacter pylori (HP) infection in screening gastric cancer and precancerous diseases. Methods:IT8 patients with upper digestive tract discomfort were selected as an observation group, and 50 healthy people d were chosen as a control group. All the cases were tested in terms of PG I, PG II and G-17 by enzyme-linked immunosorbent assay (ELISA). 14c breath tests were used to detect HP infection. Serum levels of PG, G-17 and the differences of HP infection in different diseases were compared and analyzed. Results: ( 1 ) Compared with the control group, the levels of PG I and PGR in the chronic atrophic gastritis group, the early gastric cancer group, and the advanced gastric cancer group decreased significantly,while the level of PG II increased significantly ( P 〈 0.05 ). The levels of PG I and PG II in the gastric ulcer group increased obviously, but PGR decreased ( P 〈 0.05 ). (2) Compared with the control group, G-17 levels in the gastric polyp group, the gastric ulcer group, the early gastric cancer group and the advanced gastric cancer group all increased significantly ( P 〈 0.05 ). ( 3 ) There was no significant difference in the HP infection rate among the groups ( P 〉 0.05 ). The gastrin secretion of HP positive patients in the early gastric cancer group and the advanced gastric cancer group increased significantly ( P 〈 0.05 ). Conclusion : Serum PG and G-17 can be used as important indicators for the diagnosis of gastric cancer and precancerous diseases. In gastric cancer patients,it is speculated that HP infection can affect the synthesis and release of G-17 by some means.
