摘要
目的对比分析舒芬太尼与芬太尼对中年女性患者甲状腺手术中的麻醉效果。方法将120例行甲状腺切除术的中年女性患者随机分成观察组(舒芬太尼+异丙酚)和对照组(芬太尼+异丙酚),每组60例。比较两组术前、术中、术后各时点的心率(HR)、血压(SBP、DBP)变化,以及手术前后C反应蛋白(CRP)、去甲肾上腺素(NE)、肾上腺素(E)水平变化。结果观察组术中各时间点HR、SBP、DBP与术前比较无显著差异(P> 0.05);对照组术中各时间点HR、SBP、DBP较术前升高(P <0.05),且高于同时间点观察组(P <0.05)。术后两组CRP、NE及E水平均高于术前,但观察组低于对照组(P <0.05)。结论与芬太尼相比,舒芬太尼用于中年女性甲状腺切除手术麻醉,能够减少术中心率、血压波动,有利于血流动力学稳定,并可减轻应激反应,具有较好的临床应用价值。
Objective To compare the anesthetic effects of sufentanil and fentanyl in thyroid surgery in middle-aged females. Methods A total of 120 mid-aged female patients to receive thyreoideetomy were randomly divided into an observation group (sufentanil + propofol) and a control group (fentanyl + propofol) (n=60, respectively). The changes in the heart rate (Htl) and blood pressure (SBP and DBP) at different preoperative, intraoperative and postoperative time points and the changes in the levels of C-reactive protein (CtlP), noradrenaline (NE) and epinephrine (E) before and after the operation between the two groups were compared, llesuhs There was no significant difference between the Htls, SBPs and DBPs at different intraoperative time points and those before the operation (P 〉 0.05); the Htl, SBP and DBP at intraoperative time points in the control group were higher than those before the operation (P 〈 0.05) and were higher than those at the same time point in the observation group (P 〈 0.05). The levels of postoperative CRP, NE and E in the two groups were higher than those before the operation, but the levels in the observation group were lower than those in the control group (P 〈 0.05). Conclusion Compared with fentanyl, sufentanil can reduce the fluctuation of intraoperative center rate and blood pressure, is conducive to hemodynamie stability-, can alleviate the stress response in the anesthesia in thyreoideetomy in mid-aged females, and has better clinical application value.
