摘要
目的:探讨川崎病患儿种球蛋白治疗的时机及对肿瘤坏死因子α(TNF-α)产生的影响。方法:选取2016年12月-2017年12月本院收治的川崎病患儿114例,根据丙种球蛋白使用时机的不同分为对照组和研究组,各57例。对照组在川崎病发病后使用丙种球蛋白,研究组在川崎病早期使用丙种球蛋白。对比两组患儿各项症状改善情况以及治疗前后两组患儿血清IgG1、IgG2、IgG4水平和WBC、NE、PLT、血清炎性细胞因子变化情况。结果:研究组患者退热、手足肿胀消除、住院以及黏膜充血消退时间均显著短于对照组(P<0.05)。治疗前,两组患者血清IgG1、IgG2、IgG4、WBC、NE、PLT、CRP、TNF-α、IL-6水平比较,差异均无统计学意义(P>0.05);治疗后,研究组患者血清IgG1、IgG4均较对照组明显升高,IgG2明显降低(P<0.05),WBC、NE、PLT、CRP、TNF-α、IL-6水平均较对照组显著降低,差异均有统计学意义(P<0.05)。结论:川崎病患儿早期应用丙种球蛋白进行治疗,有利于改善治疗效果,降低TNF-α水平,临床上应当推广应用。
Objective:To discuss the application timing of gamma globulin and its impact in the tumor necrosis factor-α(TNF-α) for children with Kawasaki disease.Method:114 children with Kawasaki disease treated from December 2016 to December 2017 in our hospital were selected,the children were assigned to the control group and study group according to the different application timing of gamma globulin,57 cases in each group.The control group applied the gamma globulin after the attack of the syndrome;the study group applied the gamma globulin at an earlier stage of the syndrome.The symptom improvements,the serum immunoglobulin G1(IgG1),immunoglobulin G2(IgG2) and immunoglobulin G4(IgG4) contents as well as the white blood cell(WBC),neutrophilic granulocyte(NE),platelets(PLT) and serum inflammatory cytokines were compared between the two groups.Result:The disappearance time of fever,hand-foot swelling,hospitalization and mucosal hyperemia in study group were significantly shorter than those in control group(P〈0.05).Before treatment,the levels of serum IgG1,IgG2,IgG4,WBC,NE,PLT,CRP,TNF-α,IL-6 in the two groups were compared,the differences were not statistically significant(P〉0.05);after treatment,the serum IgG1 and IgG4 contents in the study group were higher than those in control group,while the serum IgG2 contents was lower than that in control group(P〈0.05);the levels of WBC,NE,PLT,CRP,TNF-α,IL-6 were significantly lower than those of the control group,the differences were statistically significant(P〈0.05).Conclusion:The early application of gamma globulin can improve the therapeutic effect and reduce the TNF-α level,it is worthy of clinical promotion and application.
