摘要
目的:探究经尿道膀胱肿瘤电切术(TURBt)后即刻吉西他滨膀胱热灌注化疗与常温膀胱灌注化疗对老年高危非肌层浸润性膀胱癌患者的疗效。方法:选取2013年8月～2015年8月北京老年医院收治的老年高危非肌层浸润性膀胱癌患者84例,采用随机数字表的方法将研究对象分为观察组(42例)和对照组(42例),所有患者均行TURBt。观察组术后6h内行即刻吉西他滨膀胱热灌注;然后行长期膀胱热灌注;对照组术后6h内行吉西他滨常温膀胱灌注,而后行常规膀胱灌注化疗;随访比较两组患者的肿瘤复发率和不良反应发生情况。结果:全部病例随访24个月,观察组膀胱癌复发率低于对照组(P<0.05),观察组总不良反应发生率与对照组比较差异有统计学意义(42.86%vs.95.24%,P<0.05)。结论:TURBt后即刻吉西他滨膀胱热灌注化疗是预防老年高危非肌层浸润性膀胱癌患者术后复发的有效方法。
Objective：To compare the efficacy of immediate intravesical instillation of gemcitabine between hyperthermic perfusion and normal temperature perfusion after TURBt in elderly patients with high risk non-muscle invasive bladder cancer.Method：A total of 84 elderly patients with high risk non-muscle invasive bladder cancer in Beijing Geriatric Hospital from August 2013 to August 2015 were selected,the subjects were randomly divided into the observation group（42 cases）and control group（42 cases）.All patients underwent TURBt.The observation group was treated with immediate gemcitabine intravesical hyperthermic perfusion chemotherapy in 6 hours after the operation,then was treated with prolonged intravesical hyperthermic perfusion.The control group was treated with immediate gemcitabine intravesical instillation of normal temperature in 6 hours after the operation,then conventional intravesical chemotherapy was followed.The incidence of tumor recurrence rate and the adverse reaction were compared between the two groups.Result：All patients were followed up for 24 months.The bladder cancer recurrence rate of the observation group was lower than that of the control group（P〈0.05）,and the total adverse reaction rate between the observation group（42.86%）and the control group（95.24%）showed statistical significant（P〈0.05）.Conclusion：Immediate gemcitabine intravesical hyperthermic perfusion chemotherapy after TURBt is an effective method to prevent postoperative recurrence in elderly patients with high risk non-muscle invasive bladder cancer.
