摘要 目的:分析在无创呼吸机通气下有自主呼吸早产儿经细导管注入肺泡表面活性物质(pulmonary surfactant,PS)的安全性及有效性。方法:选取105例有自主呼吸早产儿分为对照组(气管插管注入PS)与观察组(细导管注入PS),比较机械通气情况、氧合情况、肺部结局。结果:观察组患儿机械通气时间、无创呼吸机使用天数、供氧时间均短于对照组(P <0. 05),观察组患儿的肺出血、气胸发生率低于对照组(P <0. 05)。结论:自主呼吸早产儿于无创呼吸机通气下经细导管注入PS具有较高安全性及有效性。 Objective: To analyze the safety and efficacy of injecting pulmonary surfactants (PS) through thin catheters for preterm infants with autonomous inspiration receiving non-invasive ventilator assistant ventilation. Methods: 105 preterm infants with autonomous respiration were chosen and divided into the control group (PS injected through tracheal intubation ) and the observation group (PS injected through thin catheters ), with the mechanical ventilation, oxygenation and lung status compared. Results: The mechanical ventilation time, the number of days of using noninvasive ventilators and the time of oxygen supply in the observation group were shorter than those in the control group( P 〈0.05). The incidence of pulmonary hemorrhage and pneumothorax in the observation group was significantly lower than that in the control group( P 〈0.05). Conclusion: Injection of PS through thin catheters into preterm infants with autonomous inspiration receiving non-invasive ventilator assistant ventilation is highly safe and effective .
