摘要 目的:分析血清肿瘤标志物及炎症因子在结直肠癌中的表达及意义。方法:112例结直肠癌患者设为观察组,60名同期体检的健康人群设为对照组,分别检测两组患者血清肿瘤坏死因子-α(TNF-α)、癌胚抗原(CEA)、糖蛋白抗原19-9(CA19-9)、C反应蛋白(CRP)、白介素-6(IL-6)及IL-8水平;探讨血清TNF-α在不同分期结直肠癌患者中的表达差异;采用受试者工作曲线分析血清TNF-α、CEA及CA19-9对结直肠癌的诊断价值。结果:观察组患者血清TNF-α水平高于对照组,差异有统计学意义(P <0. 01);观察组患者血清CEA及CA19-9水平均高于对照组,差异有统计学意义(P <0. 01);观察组患者血清CRP、IL-6及IL-8水平高于对照组,差异均有统计学意义(均P <0. 01);血清TNF-α在直肠癌患者TNM I期表达水平最低,与II~IV期比较差异均有统计学意义(均P <0. 01)。ROC曲线分析显示,以15. 64 pg/m L为截点值,TNF-α诊断结直肠癌的敏感性为71. 67%,特异性为87. 50%;以7. 306μg/L为截点值,CEA诊断结直肠癌的敏感性为85. 00%,特异性为86. 61%;以22. 360 U/m截点值,CA19-9诊断结直肠癌的敏感性为78. 33%,特异性为80. 36%。结论:血清TNF-α及肿瘤标志物CEA及CA19-9对结直肠癌具有一定的诊断价值,且TNF-α的表达水平与结直肠癌的临床分期相关,可为结直肠癌的预后预测提供参考。 Objective: To analyze the expression and clinical significance of serum tumor markers and inflammatory factors in colorectal cancer. Methods: A total of 112 patients with colorectal cancer were included in the observation group,and 60 healthy people in the same period in our hospital were used as the control group.Serum TNF -α,CEA,CA19-9 and inflammatory factors C-reactive protein (CRP),interleukin-6 (IL-6),interleukin-8 (IL-8) levels was detected in two groups.The difference of serum TNF-α expression in patients with colorectal cancer at different stages was explored.The difference in expression levels in cancer patients,the diagnostic value of serum TNF-α,CEA and CA19-9 for colorectal cancer was analyzed using the receiver operating curve. Results: The serum level of TNF-α in the observation group was significantly higher than that in the control group (P 〈0.05).The serum CEA of the observation group and CA19-9 were significantly higher than those in the control group,the difference was statistically significant ( P 〈0.05).Serum CRP,IL-6 and IL-8 in the observation group were significantly higher than those in the control group,the difference was statistically significant ( P 〈0.05).The expression of serum TNF-α was the lowest in stage I of rectal cancer,and there was significant difference between stage II and IV ( P 〈0.05).The ROC curve analysis showed that with a cut-off value of 15.64 pg/mL,the sensitivity and specificity of TNF-α in diagnosing colorectal cancer were 71.67% and 87.50%,respectively,with a cut-off value of 7.306 g/L,the sensitivity and specificity of CEA in diagnosing colorectal cancer were 85% and 86.61%,respectively,with a cut-off value of 22.360 U/m,the sensitivity and specificity of CA19-9 in diagnosing colorectal cancer were 78.33% and 80.36%,respectively.
