摘要
目的:探讨后路短节段固定联合经伤椎椎弓根螺钉固定及椎体内灌注骨水泥在治疗老年胸腰椎爆裂性骨折的可行性和中期临床疗效。方法:回顾分析采用此手术入路的患者48例,对术后随访情况,复查胸腰段X线并分析矢状位后凸Cobb角、椎管占位、伤椎前缘压缩率评估伤椎高度丢失情况。观察术后神经功能恢复(Frankel分级)及切口并发症,综合评价其可行性及中期临床疗效。结果:所有患者均获得随访,末次随访时间(30±1.2)个月。末次术后随访情况影像学资料较术前无明显丢失。神经功能均有1~2级恢复,无加重病例。结论:经伤椎置钉联合椎体成形在老年胸腰椎骨折是治疗老年性胸腰椎骨折临床疗效肯定的方法。
Objective：To study the effectiveness of Posterior short-segment fixation combined with pedicle screwsfixation and bone cement injection in injured vertebrae for the treatment of senile thoracolumbar fracture. Methods：48patients with senile thoracolumbar fractures were treated by posterior short-segment fixation combined with pediclescrews fixation and bone cement injection in injured vertebrae. Follow-up, review and analysis have been made in X-raythoracic sagittal Cobb angle of kyphosis, spinal canal, vertebral compression edge rate assessment of vertebral height losscondition. Postoperative neurologic recovery （Frankel grade） and wound complications improvement, the consolidatedinterim evaluation of its feasibility and clinical efficacy were observed. Results：All patients were followed for an averagetime of 30±1.2 months. At last year after operation, the neural function improved 1-2 grades. Conclusions：Posteriorshort-segment fixation combined with pedicle screws fixation and bone cement injection in injured vertebrae for thetreatment of senile thoracolumbar fracture the treatment of the elderly thoracolumbar fracture efficacy sure approach.
