摘要
目的探讨单纯糖尿病与合并自身免疫性甲状腺疾病患者的生化指标和胰岛自身抗体表达的差异方法.方法选取新发1型糖尿病患者81例,2型糖尿病患者157例,将合并自身免疫性甲状腺疾病的患者纳入1型糖尿病合并自身免疫性甲状腺疾病组(合并1组)、2型糖尿病合并自身免疫性甲状腺疾病组(合并2组).常规检测血糖、血脂和尿酸水平;直接化学发光法测定血清FT3,FT4,TSH,TRAb,TP0Ab,Tg Ab和C肽水平;高效液相色谱法测定糖化血红蛋白水平;应用国际标准化放射配体法检测血清GADA,1A-2A,Zn T8A,t GTA水平.结果合并1组患者年龄、空腹血糖、糖化血红蛋白明显高于单纯1型糖尿病组,TC,LDL-C水平明显低于单纯1型糖尿病组,差异具有统计学意义(P<0.05).合并2组患者中女性比例、年龄明显高于单纯2型糖尿病组,BMI值明显低于单纯2型糖尿病组,差异具有统计学意义(P<0.05).合并2组GADA+,1A-2A+,Zn T8A+率均明显高于单纯1型糖尿病组,差异具有统计学意义(P<0.05);合并1组患者任意和多种胰岛自身抗体阳性率均明显高于单纯1型糖尿病组,差异具有统计学意义(P<0.05).合并2组GADA+率、任意胰岛自身抗体阳性率明显高于单纯2型糖尿病组,差异具有统计学意义(P<0.05).结论糖尿病患者易合并自身免疫性甲状腺疾病,两者发病机制上均存在相同的免疫异常,在临床诊治中均应进行胰岛自身抗体筛查.
Objective To investigate the clinical characteristics and the expression of islet autoantibodies inpatients with diabetes mellitus and autoimmune thyroid disease. Method 81 patients with type 1 diabetesmellitus and 157 patients with type 2 diabetes mellitus were enrolled in this study. Patients with autoimmunethyroid disease were included in the group of type 1 diabetes mellitus complicated with autoimmune thyroiddisease (group 1) and type 2 diabetes mellitus complicated with autoimmune thyroid disease (group 1). Adenoiddisease group (combined with 2 groups). Blood glucose,lipids and uric acid levels were routinely measured;serum FT3,FT4,TSH,TRAb,TP0Ab,TgAb and C peptide levels were determined by direct chemiluminescencemethod;glycosylated hemoglobin levels were determined by high performance liquid chromatography; serumGADA,1A-2A, ZnT8A and tGTA levels were determined by international standardized radioligand method.Results The age,fasting blood glucose and glycosylated hemoglobin of the patients with type 1 diabetes mellituswere significantly higher than those of the patients with type 1 diabetes mellitus,and the levels of TC and LDL-Cwere significantly lower than those of the patients with type 1 diabetes mellitus(P〈0. 05). The proportion and ageof women in the combined group were significantly higher than those in the simple type 2 diabetes mellitus group,and the BMI value was significantly lower than that in the simple type 2 diabetes mellitus group (P〈0. 05). Therates of GADA+,IA-2A+ and ZnT8A+ in group 1 were significantly higher than those in type 1 diabetes mellitusalone (P〈0. 05),and the positive rates of arbitrary and multiple islet autoantibodies in group 1 were significantlyhigher than those in type 1 diabetes mellitus alone (P〈0. 05). The GADA+ rate and positive rate of arbitraryislet autoantibodies in the two groups were significantly higher than those in the simple type 2 diabetes mellitusgroup (P〈0. 05).
