摘要
目的分析采用无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的实际效果。方法选择2017年1月～2018年1月间在我院进行治疗的慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者共90例作为研究分析对象,采用随机数字法将所有患者分为观察组与对照组两组,其中对照组患者采用常规方式进行治疗,观察组患者在对照组的治疗基础上采用无创呼吸机治疗,将两组患者的治疗总有效率、不良反应出现率及pH值、PaO_2、PaCO_2变化进行比较。结果在不良反应出现率方面,两组之间无明显差异(P>0.05),在治疗总有效率方面,观察组明显高于对照组(P<0.05),在pH值、PaO_2、PaCO_2的改善程度方面,观察组明显优于对照组,差异有统计学意义(P<0.05)。结论在对慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的治疗中,使用无创呼吸机有显著的效果,同时具有一定的安全性,值得在临床治疗中推广应用。
Objective To analyze the practical effect of non-invasive ventilator in the treatment of patients with chronic obstructive pulmonary disease（COPD） complicated with type II respiratory failure. Methods A total of 90 patients with chronic obstructive pulmonary disease （COPD） and type II respiratory failure who were treated in our hospital from January 2017 to January 2018 were enrolled in the study. All patients were divided into observation group and control group by random number method. The control group was treated by conventional methods, and the observation group was treated with non-invasive ventilator on the basis of the treatment of the control group. The total effective rate of treatment, the incidence of adverse reactions, the pH, PaO2, and PaCO2 changes between the two groups were compared. Results There was no significant difference in the incidence of adverse reactions between the two groups （P〉0.05）. The total effective rate of treatment in the observation group was significantly higher than that of the control group （P〈0.05）. The improvement of pH, PaO2 and PaCO2 in the observation group was significantly better than that of the control group （P〈0.05）, and the difference was statistically significant. Conclusion In the treatment of patients with chronic obstructive pulmonary disease（COPD） and type II respiratory failure, the use of non-invasive ventilator has a significant effect, while having a certain degree of safety, it is worthy of popularization and application in clinical treatment.
