肥胖男性精浆同型半胱氨酸、叶酸和维生素B12浓度与精液参数的相关性分析 被引量:6

摘要 目的:探讨肥胖男性精浆同型半胱氨酸(Hcy)及其代谢相关因子叶酸(FA)和维生素B12(VB12)与精液参数的相关性。方法:随机选取83例特发性不育男性为研究对象,依据体质量指数(BMI)分为正常组(BMI <24 kg/m^2,n=28)、超重组(24 kg/m^2≤BMI <28 kg/m^2,n=28)、肥胖组(BMI≥28 kg/m^2,n=27),采用ELISA法检测不同BMI组精浆Hcy、FA及VB12的浓度,并与精液参数进行相关性分析。结果:与正常组相比,肥胖组男性精液量[(2. 58±0. 77) ml vs (3. 23±0. 86) ml]、精子总数[(121. 55±88. 08)×106vs (191. 35±103. 00)×106]、前向运动精子百分率[PR%,(38. 97±16. 52)%vs (52. 88±15. 58)%]、精浆Hcy[(6. 62±0. 85)μmol/L vs (7. 41±1. 29)μmol/L]、VB12[(230. 07±37. 75) pmol/L vs (282. 41±30. 38) pmol/L]显著降低(P <0. 05),超重组各项精液参数与正常组和肥胖组均无显著差异(P> 0. 05);精子浓度和精浆FA浓度3组间无统计学差异(P> 0. 05)。相关性分析发现,精浆Hcy、VB12浓度与精液量(r=0. 281、0. 242)和精子总数(r=0. 229、0. 258)呈正相关(P <0. 05),与BMI(r=-0. 293、-0. 238)呈负相关(P <0. 05),而与精子浓度和PR%无相关性(P> 0. 05)。精浆FA浓度与BMI、精液参数无相关性(P> 0. 05)。结论:精浆Hcy、VB12与BMI、精液量和精子总数存在相关性,推测精浆Hcy和VB12可能对肥胖男性的生育力有一定影响。 Objective: To investigate the correlation of seminal plasma homocysteine (Hcy) and its metabolic factors folate (FA) and eobalamin (VB12) with semen quality in obese men. Methods: We randomly selected 83 male patients with idiopathic infertility for this study and, according to the body mass index (BMI), divided them into a normal BMI (n=28), an overweight (n=28) and an obesity group (n=27). We determined the levels of Hcy, FA and VB12 in the seminal plasma by ELISA and ana- lyzed their correlation with the semen parameters of the patients in different groups. Results : Compared with the normal BM1 group, the obese males showed significant decreases in the semen volume ([3.23 ± 0.86 ] vs [2.58±0.77 ] ml, P 〈 0.05 ), total sperm count ([191.35± 103.00] vs [121.55± 88.08]×10^6, P 〈 0.05), percentage of progressively motile sperm ([52.88±15.58] % vs [38.97 ±16.52] %,P 〈 0.05), seminal plasma Hcy ( [ 7. 41 ±1.29] vs [6.62± 0.85] μmol/L,P 〈 0.05), and VB12 ( [ 282.41±30.38 ] vs [ 230.07±37.75 ] pmol/L, P 〈 0.05 ). There were no statistically significant differences in the semen parameters between the overweight group and the normal BMI or the obese group ( P 〉 0.05 ), or in sperm concentration or the FA level among the three groups of patients (P 〉 0.05 ). The levels of seminal plasma Hcy and VB12 were correlated positively with the semen volume ( r = 0. 281 and 0. 242, P 〈 0.05 ) and total sperm count ( r = 0.229 and O. 258, P 〈 O. 05 ) but negatively with BMI (r = -0. 293 and -0. 238, P 〈 0.05). No correlation, however, was found either between sperm concentration and the percentage of progressively motile sperm (P 〉 0.05 ) or between the FA level and BMI or semen parameters (P 〉 O. 05 ). Con- clusion: The levels of seminal plasma Hcy and VB12 are correlated with BMI, semen volume and total sperm count, which suggests that the concentrations of seminal plasma Hcy and VB12 may be associated with the
