右美托咪定复合丙泊酚对急诊脑外伤手术麻醉患者炎性反应及血流动力学的影响 被引量:8

Effect of Dexmedetomidine Combined with Propofol on Inflammatory Response and Hemodynamics in Patients Undergoing Emergency Cerebral Trauma Surgery Under Anesthesia

摘要 目的探讨右美托咪定复合丙泊酚对急诊脑外伤手术麻醉患者炎性反应及血流动力学的影响。方法选取2015年2月至2017年2月于医院接受急诊脑外伤手术的患者70例,按随机数字表法分成A组与B组,各35例。A组予丙泊酚乳状注射液麻醉,B组予右美托咪定复合丙泊酚麻醉。结果术后24 h,B组血清白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于A组,IL-10水平高于A组(P <0. 05); B组收缩压、舒张压及心率水平气管插管即刻(T2)、切皮时(T3)、拔管后(T4)均低于A组(P <0. 05); B组自主呼吸恢复时间、苏醒时间低于A组(P <0. 05); B组呛咳、躁动发生率均为2. 86%,明显低于A组的22. 86%和20. 00%(P <0. 05)。结论右美托咪定复合丙泊酚可有效减轻急诊脑外伤手术患者的炎性反应,且能有效维持血流动力学稳定,缩短自主呼吸恢复时间与苏醒时间,安全性较好,值得临床推广。 Objective To investigate the effects of dexmedetomidine combined with propofol on inflammatory response and hemodynamics in patients undergoing emergency cerebral trauma surgery under anesthesia. Methods Totally 70 patients undergoing emergency cerebral trauma surgery in our hospital from February 2015 to February 2017 were selected and divided into group A and group B according to the random number table method, 35 cases in each group. The group A was anesthetized with Propofol Emulsion Injection and group B was given dexmedetomidine combined with propofol for anesthesia. Results 24 h after operation, the levels of serum interleukin-6(IL-6) and tumor necrosis factor- c^(TNF- c^) in group B were lower than those in group A, while the level of IL- 10 in group B was higher than that in group A(P 〈 0.05).The levels of systolic blood pressure(SBP), diastolic blood pressure(DBP) and heart rate(HR) in group B were lower than those in group A at the moment of endotracheal intubation(T2), skin incision(T3) and after extubation(TO (P 〈 0. 05). The time of spontaneous respiration restoration and recovery time in group B was shorter than that in group A(P 〈 0.05). The incidence rates of irritating cough and restlessness in group B both were 2.86%, which were lower than 22.86% and 20. 00% in group A(P 〈 0.05). Conclusion Dexmedetomidine combined with propofol can eft^ctively alleviate the inflammatory response of patients undergoing emergency cerebral trauma surgery, and eft^ctively maintain hemodynamic stability, shorten the time of spontaneous respiration restoration and recovery time. It is safe and worthy of clinical promotion.
