摘要
目的分析因中枢神经系统疾病死亡患者的临终心电图特点。方法 27例因中枢神经系统疾病死亡患者作为研究组, 27例因其他疾病所致死亡患者作为对照组。比较两组患者的临终心电图表现。结果研究组死亡原因为脑梗死的患者18例,其余为脑出血、中枢神经系统感染等疾病。心电图表现均有不同类型的临终心电图改变,其中以混合型最多见,发生率为70.37%(19/27),室性心动过速-心室颤动型、停搏型和阻滞型的发生率分别为7.41%(2/27)、11.11%(3/27)、11.11%(3/27)。对照组主要死亡原因为内科各种疾病。心电图表现亦有不同类型的临终心电图改变,其中以混合型最多见,发生率为62.96%(17/27),室性心动过速-心室颤动型、停搏型和阻滞型的发生率分别为11.11%(3/27)、11.11%(3/27)、14.81%(4/27)。两组临终心电图改变分型比较,差异无统计学意义(P>0.05)。结论因中枢神经系统疾病死亡患者的临终心电图演变仅是一个结果 ,并非直接死亡原因,一旦发生,各种抢救措施往往效果不佳,预防的关键是原发病的治疗。对于因严重中枢神经系统疾病步入临终结局的患者,减少无意义抢救,推行脑死亡的临床判定标准有重要意义。
Objective To analyze the characteristic of deathbed electrocardiogram in patients with central nervous system diseases. Methods A total of 27 patients died of central nervous system diseases were selected as the research group, and 27 patients died of other diseases were selected as the control group. The manifestations of deathbed electrocardiogram between the two groups were compared. Results The death causes of 18 patients in the research group were cerebral infarction, the rest were cerebral hemorrhage, central nervous system infection and other diseases. Electrocardiogram manifestations showed different types of deathbed electrocardiogram changes, of which mixed type was the most common, the incidence rate was 70.37%(19/27), the incidence of ventricular tachycardia-ventricular fibrillation type, arrest type and block type were 7.41%(2/27), 11.11%(3/27) and 11.11%(3/27) respectively. The main causes of death in the control group were various internal diseases. Electrocardiogram showed different types of deathbed electrocardiogram changes, of which the mixed type was the most common, the incidence rate was 62.96%(17/27), the incidence of ventricular tachycardiaventricular fibrillation type, arrest type and block type were 11.11%(3/27), 11.11%(3/27) and 14.81%(4/27) respectively. There were no significant differences between the two groups in the classification changes of deathbed electrocardiogram(P〉0.05). Conclusion The evolution of deathbed electrocardiogram in patients with central nervous system disease is only a result, not a direct cause of death. Once it happens, various rescue measures are often ineffective. The key to prevention is the treatment of the primary disease. It is of great significance to reduce the number of meaningless rescues and carry out the clinical criteria for determining brain death in patients with severe central nervous system diseases who have entered the terminal stage.
