摘要
目的:观察不同施灸温度对脑卒中患者肢体痉挛疗效的差异,为患者提供适宜、客观的施灸温度。方法:选取患者80例,采用随机数字表法分为热灼感组和温感组各40例,热灼感组给予36.8~43.5℃施灸,温感组给予32.8~35.8℃施灸,每次施灸30 min,1次/d,5次/疗程,持续干预4个疗程。选用改良Ashworth、简化Fugl-Meyer运动功能、Barthel指数量表进行评定,采用意向性分析统计方法。结果:干预4个疗程末与干预前比较,2组患者上肢痉挛程度有所改善,差异有统计学意义(P<0.05),热灼感组更为明显,组间比较,时间主效应具有统计学意义(P<0.05),处理主效应无统计学意义(P>0.05);干预4个疗程末,2组患者运动功能、日常生活能力评分与干预前差值均大于相应的最小临床意义变化值,差异具有统计学意义(P<0.05),热灼感组效果更佳。结论:2种施灸温度干预脑卒中肢体痉挛患者均能有效改善肢体痉挛,提高运动功能和日常生活能力;热灼感组疗效优于温感组,不良事件发生率低。
Objective: To evaluate therapeutic effects of different moxibustion temperatures on stroke patients with spastic hemiplegia, in order to provid suitable and objective moxibustion temperature for such patients with abnormal perceptual abnormalities.Methods: A total of 80 patients were randomly divided into thermal burning group and temperature-sensing group, with 40 cases in each group. The thermal burning group received moxibustion at 36.8-43.5℃ and the temperature-sensing group received moxibustion at 32.8-35.8℃, 30 minutes each course, one course a day and five courses a week, continuous treatment for four weeks. A modified MAS scale was used to assess the degree of spasticity; a simplified Fugl-Meyer scale was used to assess motor function; Barthel's number was used to assess the ability of daily living. Results: Comparing the difference at the end of intervention to that before, the degree of upper limb paralysis was improved in both groups with statistically significant difference(P〈0.05), the effect of thermal burning group improved more obvious. Comparison of two groups showed that the major effects of time were statistically different(P〈0.05),but the major effects of intervention with no significant different(P〉0.05). After four treatments, two groups of motor function, daily life ability were greater than the corresponding minimum clinically important difference, with statistical significance(P〈0.05), thermal burning groups was more obvious. Conclusion: Different moxibustion temperature has a certain therapeutic effect on limb paralysis in patients with stroke,which can improve physical paralysis effectively, improve motor function and daily life ability, the thermal burning group is superior to the temperature-sensing group, and with low incidence of adverse events is low.
