摘要
目的分析降钙素原(PCT)与C反应蛋白(CRP)在肺炎合并脓毒血症患者的病情严重程度以及预后评价中的作用。方法随机选取2012年5月-2017年6月天津市第三中心医院收治的肺炎合并脓毒血症患者85例为观察组,根据病情程度不同,分为一般脓毒症组(28例)、严重脓毒症组(30例)及脓毒性休克组(27例);同期收治的单纯肺炎患者55例为对照组。比较患者血清PCT、CRP水平及Apache-II评分情况。结果观察组血清PCT、CRP水平及Apache-II评分值高于对照组,差异有统计学意义(P<0.05);随着患者病情严重程度的加重,患者血清PCT、CRP水平及Apache-II评分值逐渐升高,差异有统计学意义(P<0.05);入院3周内,死亡组患者血清PCT、CRP水平及Apache-II评分高于存活组,差异有统计学意义(P<0.05)。结论血清PCT、CRP水平可作为评价肺炎合并脓毒血症患者病情严重程度及预后情况的指标。
Objective To analyze the importance of procalcitonin(PCT) and C-reactive protein(CRP) in assessing the severity of pneumonia and sepsis patients as well as prognostic evaluation. Methods A total of 85 patients with pneumonia complicated with sepsis were randomly selected from May 2012 to June2017 in Tianjin Third Central Hospital as observation group,they were divided into three groups according to severity,namely sepsis group(n=28),severe sepsis group(n =30) and septic shock group(n =27). And in the same period,55 patients with simple pneumonia were enrolled as control group. The serum PCT,CRP level and Apache-II scores case were compared. Results The levels of serum PCT,CRP and Apache-II in observation group were higher than those in control group,the difference was statistically significant(P〈0.05). With the aggravation of patients' severity,serum PCT,CRP levels and Apache-II scores gradually increased,and the differences were statistically significant(P〈0.05). Within 3 weeks of admission,the levels of PCT,CRP level and Apache-II scores in the death group were higher than those in the survival group,and the differences were statistically significant(P0.05). Conclusion Serum PCT and CRP levels can be used as indicators for evaluating the severity and prognosis of patients with pneumonia combined with sepsis.
