摘要
目的探讨加速康复外科(ERAS)在老年患者急诊上消化道穿孔修补术中的应用价值。方法选取2016年6月至2018年6月该院收治的老年急诊上消化道穿孔修补术患者80例(年龄均大于或等于60岁,均取得患者家属知情同意),采用随机字数表法分为治疗组和对照组,每组40例。治疗组应用ERAS理念治疗,对照组应用常规围手术期理念治疗。比较两组患者血清降钙素原、C反应蛋白、住院时间、首次下床活动时间、术后疼痛程度(疼痛视觉模拟评分大于5分病例数)、手术总体并发症等。结果两组患者年龄、性别等一般资料比较,差异均无统计学意义(P>0.05);治疗组患者术后血清降钙素原、C反应蛋白、住院时间、下床活动时间、术后疼痛程度、手术总体并发症发生率均明显低于对照组,差异均有统计学意义(P<0.05)。结论 ERAS理念用于老年患者急诊上消化道穿孔修补术中具有一定的优势,值得推广应用。
Objective To investigate the application of enhanced recovery after surgery(ERAS)in the treatment of gastrointestinal perforation repair in elderly patients. Methods 80 elderly patients(aged 60 or more with informed consent)with upper gastrointestinal perforation repair in the hospital from June 2016 to June 2018 were selected and divided into the treatment group and the control group according to random number table,40 cases in each group,and the reatment group was applied with ERAS,while the control group was treated with conventional erioperative concept. The serum procalcitonin (PCT),C-reactive protein(CRP),hospital stays,leaving bed time after operation,postoperative pain degrees(proportion with VAS score greater than 5),overall postoperative complications and so on were compared between the two groups. Results Comparison between the treatment group and the control group showed no statistically significant difference in age,gender and other general data(P〉0.05). The PCT level,CRP level,hospital stays,leaving bed time,postoperative pain degrees and occurrence rate of overall postoperative complications in the treatment group were obviously lower than those in the control group,and the difference had statistical significance(P〈0.05). Conclusion The application of ERAS is advantageous in the emergency treatment of gastrointestinal perforation in elderly atients,and it is worthy of promotion and application.
