摘要
目的探讨标准外伤去大骨瓣减压术在急性硬膜下血肿患者中应用效果。方法选取2015年7月—2017年8月收治的83例急性硬膜下血肿患者,依据手术术式不同随机分组,对照组41例采用常规骨瓣开颅术治疗,观察组42例实施标准外伤去大骨瓣减压术治疗,观察比较两组手术前后格拉斯哥昏迷指数(GCS)评分及术后不同时间点(1、3、5 d)颅内压变化情况,并统计两组并发症发生情况。结果术后观察组GCS评分高于对照组,差异具有统计学意义(P<0.05);观察组术后1、3、5 d颅内压均小于对照组,差异具有统计学意义(P<0.05);观察组并发症发生率为4.76%(2/42),小于对照组21.95%(9/41),差异具有统计学意义(P<0.05)。结论标准外伤去大骨瓣减压术可降低急性硬膜下血肿患者颅内压,改善昏迷状况,降低并发症发生率。
Objective To investigate the application effect of the standard decompressive craniectomy for large trauma in patients with acute subdural hematoma. Methods A total of 83 patients with acute subdural hematoma treated in our hospital from July 2015 to August 2017 were selected and divided into different groups according to the operation methods. The control group（ n = 41） was treated with the conventional bone flap craniotomy,and the observation group（ n = 42） was treated with the standard decompressive craniectomy for large trauma. The Glasgow coma index（ GCS） score and the change of intracranial pressure at different time points（ 1 d,3 d and 5 d） after operation were observed and compared between the two groups. The complications of the two groups were statistically analyzed. Results The GCS score of the observation group was higher than that of the control group after operation（ P〈 0. 05）. The intracranial pressure at 1 d,3 d and 5 d after operation in the observation group were less than those in the control group（ P〈 0. 05）. The complication rate of the observation group was 4. 76%（ 2/42）,which was less than that of the control group 21. 95%（ 9/41）（ P〈 0. 05）.Conclusion The standard decompressive craniectomy for large trauma can reduce the intracranial pressure,improve the coma,and reduce the incidence of complications in patients with acute subdural hematoma.
