摘要
目的比较超声刀和电凝钩在经脐单孔腹腔镜胆囊切除术中的应用效果。方法采用前瞻性对照研究方法,将2016年1月至2017年9月在上海市杨浦区中心医院普外科收治的150例非急性结石性胆囊炎患者随机分为两组,分别采用超声刀和电凝钩行经脐单孔腹腔镜胆囊切除术,对比分析两组患者的手术相关指标、术后恢复指标等。结果所有患者均顺利完成经脐单孔腹腔镜胆囊切除术,无中转常规腹腔镜手术或开腹手术。超声刀组和电凝钩组在手术时间、术中出血量和术中胆囊破损发生率上差异无统计学意义(均P>0.05);超声刀组在术后住院时间[(2.6±1.5)d]、术后24 h疼痛评分和术后并发症发生率方面明显优于电凝钩组(均P<0.05)。结论经脐单孔腹腔镜胆囊切除术安全可行,使用超声刀进行该手术的效果要优于电凝钩,术后并发症明显减少。
Objective To compare the effect of ultrasonic knife and electrocoagulation hook in the laparoscopic cholecystectomy with single hole. Methods Prospective comparison study method,in January 2016 to September 2016 in the center of the Shanghai yangpu district general hospital treated 150 cases of acute calculous cholecystitis patients were randomly divided into two groups,respectively,using ultrasonic knife and electric coagulation hook through the umbilical single-arch laparoscopic cholecystectomy,comparative analysis of two groups of patients with indicators related to the operation and postoperative recovery index,etc. Results All patients underwent laparoscopic cholecystectomy without conventional laparoscopic surgery or laparoscopic surgery. Ultrasonic knife and electric coagulation hook group in operation time,intraoperative blood loss,and there is no statistical difference of intraoperative gallbladder breakage rate（ P〈 0. 05）,but the ultrasonic knife set in the postoperative hospital stay,postoperative pain score and the incidence of postoperative complications in 24 hours to electric coagulation hook is obviously better than the other group（ all P〈 0. 05）. Conclusion the laparoscopic cholecystectomy is safe and feasible,and the effect of using ultrasonic knife is better than that of electrocoagulation hook,and the postoperative complications are significantly reduced.
