摘要
目的观察百令胶囊联合阿拓莫兰治疗儿童急性肾损伤的疗效。方法将52例急性肾损伤患儿随机分为常规组和联合组,每组26例。常规组给予对症治疗并静滴阿拓莫兰,联合组在常规组治疗基础上加服百令胶囊,疗程均为28 d。检测2组患儿治疗前及治疗3 d、7 d、14 d和28 d血尿素(Urea)、肌酐(SCr)、胱抑素C(Cys C)、β2微球蛋白(β2-MG)和24 h尿微量白蛋白(U-MA)、尿渗透压,计算内生肌酐清除率(Ccr)及尿微量白蛋白排泄率(UAER),统计2组临床疗效及少尿期、多尿期持续时间和SCr恢复正常时间。结果从治疗第3天起,2组Urea、SCr、U-MA、Cys C、β2-MG、UAER均逐渐降低(P均<0. 05),Ccr、尿渗透压均逐步升高(P均<0. 05),且联合组治疗7,14和28 d时Urea、SCr、U-MA、Cys C水平和治疗14,28 d时β2-MG、UAER均明显低于同期常规组(P均<0. 05),Ccr、尿渗透压均高于同期常规组(P均<0. 05)。联合组总有效率明显高于常规组(P <0. 05),少尿期持续时间、多尿期持续时间及SCr恢复正常时间均明显短于常规组(P均<0. 05)。结论百令胶囊与阿拓莫兰治疗儿童急性肾损伤具有协同作用,联合应用可增强药物疗效,可明显缩短多尿期、少尿期持续时间及SCr恢复正常时间,对肾脏具有明显保护作用。
Objective It is to observe the curative effect of Atomolan combined with Bailing Capsule on acute kidney injury in children and explore its mechanism. Methods 52 cases of children with acute kidney injury were randomly divided into routine treatment group and combined treatment group,26 cases in each group. The conventional treatment group mainly adopted symptomatic treatment and was injected Atomolan intravenously,on the basis of routine treatment,combined treatment group was treated with Bailing Capsule. Both groups were treated for 4 weeks. Blood urea（ Urea）,creatinine（ SCr）,Cystatin C（ Cys C）,β2 microglobulin（ β2-MG） and 24 h urine microalbumin（ U-MA）,urine osmotic pressure of all the patients were measured in both groups before treatment and at 3,7,14 and 28 days after treatment,endogenous creatinine clearance（ Ccr） and urine microalbumin excretion rate（ UAER） were calculated,the clinical efficacy was evaluated,duration of polyuria and oliguria,recovery time of SCr were recorded. Results From the third day of treatment,the Urea,SCr,U-MA,Cys C,β2-MG,and UAER of the two groups were gradually decreased（ P〈0. 05）,and the Ccr and urine osmotic pressure were gradually increased（ P〈0. 05）,furthermore,the levels of Urea,SCr,U-MA,Cys C andβ2-MG and UAER at 14 and 28 days were significantly lower in combined treatment group than those in the conventional group at the 14 th,14 th and 28 th day after treatment（ P〈0. 05）,Ccr,Urinary osmotic pressure was higher than those in the conventional group（ P〈0. 05）. The total effective rate of the combined treatment group was significantly higher than that of the conventional group（ P〈0. 05）. The duration of oliguria,duration of polyuria and normal time of SCr were significantly shorter in combined treatment group than those in the conventional group（ P〈0. 05）.
