摘要
目的探讨不同体位麻黄碱滴鼻预防高压氧所致中耳气压伤的临床效果。方法随机选取2014年1月~2017年11月我院收治的首次行高压氧治疗的60例患者作为研究对象,随机将其分为观察组(n=30)和对照组(n=30)。两组患者同舱治疗,第1、2、3天进舱前予麻黄碱滴鼻,对照组患者采用头后仰平卧位滴麻黄碱,观察组患者采用平卧位头后仰侧转45°体位滴麻黄碱。比较两组患者的中耳气压伤发生情况、鼓膜损伤程度及高压氧治疗耐受性,分析评估不同体位麻黄碱滴鼻对高压氧治疗所致中耳气压伤的预防效果及患者耐受性的影响。结果观察组患者的中耳气压损伤发生率(6.66%)显著低于对照组(26.66%),差异有统计学意义(P<0.05)。观察组患者的鼓膜损伤程度明显轻于对照组,差异有统计学意义(P<0.05)。观察组患者的高压氧治疗耐受度(93.33%)明显高于对照组(73.33%),差异有统计学意义(P<0.05)。结论麻黄碱滴鼻时选择平卧位头后仰侧转45°体位,有利于降低患者的中耳气压损伤发生率,减轻鼓膜损伤程度,提高其高压氧治疗耐受性,在预防高压氧所致中耳气压伤的应用中,效果显著,值得临床借鉴和推广。
Objective To investigate the clinical effect of Ephedrine Nasal Drops with different postures in the prevention of barotrauma of the middle ear caused by hyperbaric oxygenation. Methods A total of 60 patients who underwent the hyperbaric oxygen therapy for the first time in our hospital from January 2014 to November 2017 were selected as objects. They were randomly divided into observation group(n=30) and control group(n=30). The two groups of patients were treated in the same capsule, and the Ephedrine Nasal Drops were used before entering the capsule on the 1 st, 2 nd and 3 rddays. Patients in the control group received Ephedrine on the supine position, while head-side in 45° on the horizontal position was adopted in the observation group. The occurrence of barotrauma of the middle ear, the extent of tympanic membrane damage, and hyperbaric oxygen therapy tolerance were compared in order to analyze and evaluate the effect of different positions of Ephedrine Nasal Drops on the prevention of barotrauma of the middle ear caused by hyperbaric oxygen therapy and the influence on patient′s tolerance. Results The incidence of the middle ear barotrauma in the observation group was 6.66%, which was significantly lower than that in the control group accounting for26.66%, and the difference was statistically significant(P〈0.05). The degree of tympanic membrane damage was significantly lighter than that in the control group, and the difference was statistically significant(P〈0.05). The tolerance of hyperbaric oxygen treatment in the observation group was 93.33%, which was significantly higher than that in the control group accounting for 73.33%, and the difference was statistically significant(P〈0.05). Conclusion Ephedrine Nasal Drops in patients with head-side in 45° on the horizontal position is helpful to reduce the incidence of barotrauma of the middle ear, alleviate the degree of tympanic membrane damage, and increase the patients′ tolerance to hyperbaric oxygen therapy, which obtains a
