摘要
目的探讨直接前侧入路微创全髋关节置换术的早期疗效。方法选取2015年11月~2016年12月期间在我院进行单侧人工全髋关节置换术95例,采用随机数字表法将其分为两组,观察组(n=48)和对照组(n=47)。观察两组手术情况、术后情况、术后并发症发生率及Harris评分。结果观察组患者的切口大小,术中出血量,手术时间,术后下床活动时间及住院时间皆低于对照组(P <0.05)。两组患者Harris评分术后得分皆高于术前(P <0.05),两组患者术后Harris评分比较,差异无统计学意义(P> 0.05)。观察组患者术后并发症总发生率为6.25%,明显低于对照组的14.90%,两组比较差异显著(P <0.05)。结论直接前侧入路微创全髋关节置换术的早期疗效显著,髋关节功能恢复良好,不良反应发生率低,值得在临床上进一步推广和应用。
Objective To explore the early effect of minimally invasive total hip arthroplasty through direct anterior approach. Methods From November 2015 to December 2016, ninety-five eases of unilateral total hip arthroplasty were randomly divided into two groups： observation group（48 eases） and eontrol group（47 eases）. The operation conditions, postoperative conditions, postoperative complications and Harris score of two groups were observed. Results The incision size, intraoperative bleeding volume, operation time, postoperative ambulation time and hospitalization time of the observation group were lower than those of the control group（P 〈 0.05）. The postoperative Harris scores of the two groups were higher than those before operation（P 〈 0.05）, there was no significant difference between the two groups（P 〉 0.05）. The total incidence of postoperative complications in the observation group was 6.25%, which was significantly lower than that in the control group（14.90%）,P 〈 0.05. Conclusion Direct anterior approach minimally invasive total hip arthroplasty has a significant early effect, good reeovery of hip function, low incidence of adverse reactions, and is worthy of further promotion and application in clinic.
