多指标联合检测对乙型肝炎性肝硬化的相关性研究及预后分析

摘要 目的探究血清碱性磷酸酶(ALP)、血糖及尿微量蛋白联合检测对乙型肝炎性肝硬化的相关性及预后影响。方法选取140例乙型肝炎性肝硬化患者为观察组,57例身体各项指标正常的志愿者为对照组,分别检测其血清ALP、血糖及尿微量蛋白水平并进行比较。不同肝硬化分级患者血清ALP、血糖及尿微量蛋白的Logistic回归分析;比较两组患者并发症发生情况。结果在不同肝硬化分级中,A组患者血清ALP水平最高,B组逐步降低,C组再次升高;随着肝硬化分级的增加,患者血糖、尿微量蛋白水平呈梯度增高;不同组间患者的血清ALP、尿微量蛋白水平均存在显著差异(P<0.05)。血清ALP、血糖及尿微量蛋白水平均是乙型肝炎性肝硬化病情程度的独立影响因素(P<0.05)。观察组并发症总发生率显著高于对照组(P<0.05)。结论血清ALP、血糖及尿微量蛋白联合检测可有效地诊断乙型肝炎性肝硬化的发生及其严重程度,对疾病的治疗有重要的临床参考意义。 Objective To investigate the correlation and prognosis influence of serum alkaline phosphatase (ALP), bloodglucose and urinary microprotein in patients with posthepatitic B cirrhosis. Methods A total of 140 patients with posthepatiticB cirrhosis were selected as observation group and 57 healthy volunteers as control group, the levels of serum ALP, bloodglucose and urine microprotein were detected respectively and compared. Logistic regression analysis of serum ALP, bloodglucose and urinary microprotein in patients with different liver cirrhosis grades was performed. The incidences ofcomplications between the two groups were compared. Results In different liver cirrhosis grades, the serum ALP level ofgroup A was the highest, that of the groupB gradually decreased, and increased again in the group C, with the increase ofliver cirrhosis grades the blood glucose and urine microprotein levels were gradiently increased. The levels of serum ALP andurine microprotein in different groups were significantly different(P〈0.05). The levels of serum ALP, blood glucose andurinary microalbumin were independent influencing factors for the severity of posthepatitic B cirrhosis (P〈0.05). The totalincidence of complications in the observation group was significantly higher than that of the control group (P〈0.05).Conclusion The combined detection of serum ALP, blood glucose and urinary microprotein can effectively diagnose theoccurrence and severity of posthepatitic B cirrhosis, which has important clinical significance for the treatment of the disease.
