膝骨关节炎患者在太极站桩练习中髌骨位置对膝关节应力的影响 被引量:11

Effects of different patellar position on knee loading during Taichi Zhanzhuang exercise in patients with knee osteoarthritis

摘要 目的:研究膝骨关节炎(knee osteoarthritis,KOA)患者在太极站桩练习过程中髌骨位置的变化对膝关节应力的影响。方法:纳入30例早期KOA患者,采用VICON三维步态采集分析系统配合4块AMTI三维测力台采集KOA患者在髌骨平行于同侧足尖、髌骨平行于同侧足尖后2 cm、髌骨平行于同侧足尖前2 cm这3种不同条件下膝关节的运动学、动力学参数。结果:与其余两种条件比较,髌骨平同侧足尖前2 cm时膝关节的最大屈曲角度、内收力矩及屈曲力矩均显著增高(P<0.05);而髌骨平同侧足尖后2 cm与髌骨平同侧足尖时膝关节的内收力矩和屈曲力矩比较,其差异均无统计学意义(P>0.05)。结论:KOA患者在太极站桩练习过程中当髌骨位置超过同侧足尖时,膝关节矢状面及冠状面的受力会显著增加。 Objective: To investigate the effects of different patellar position on knee loading of patients with knee osteoarthritis( KOA) during Taichi Zhanzhuang exercise. Methods: Thirty patients with KOA at early stage were enrolled. The kinematics and kinetics parameters were collected by VICON Nexus three dimensional motion analysis system and four AMTI force platforms under three conditions((1)patellar position was paralleled to the ipsilateral large toe,(2)patellar position was 2 cm behind the ipsilateral large toe,(3)patellar position was 2 cm over the ipsilateral large toe). Results: Compared with the other two conditions,the maximum flexion angle,adduction moment and flexion moment of knee in the condition that patellar position was 2 cm over the ipsilateral large toe were obviously increased( P〈0.05); while there were no statistically significant differences on the adduction moment and flexion moment of knee between the different conditions that patellar position was paralleled to the ipsilateral large toe and patellar position was 2 cm behind the ipsilateral large toe( P〈0.05). Conclusion: During the Taichi Zhanzhuang exercise,when the patellar position is over the ipsilateral large toe,the loading of the knee on sagittal and frontal plane in KOA patients is significantly increased.
