摘要
目的分析在糖调节异常(impaired glucose regulation, IGR)者中血钾水平对胰岛β细胞功能的影响,可能为这类患者的治疗提供新的方法。
方法选取320例IGR,所有人群均行口服75 g葡萄糖耐量试验,收集空腹血样本及糖负荷后30min、120min血样本,并对入选者进行问卷调查、体格检查、胰岛素及血钾相关指标的测定,计算评价胰岛β细胞功能的参数(DI0、DI30、DI120)。
结果在IGR中,DI0与体重指数(body mass index, BMI)(r=-0.122,P=0.015)、总胆固醇(total cholesterol, TC)(r=-0.182,P=0.032)、低密度脂蛋白-胆固醇(low density lipoprotein- cholesterol, LDL-C)(r=-0.178,P=0.026)和甘油三酯(triglyceride,TG)(r= -0.179,P=0.011)呈负相关;DI30与TC(r=-0.146,P=0.001)、TG(r=-0.192,P=0.027)和LDL-C(r=-0.134,P=0.014)呈负相关;DI120与TC(r=-0.222,P〈0.001)、TG(r=-0.209,P〈0.001)和LDL-C(r=-0.183,P〈0.001)呈负相关,与血钾(r=0.208,P〈0.001)呈正相关。多元逐步回归分析提示,在校正血脂、年龄等危险因素后,血钾水平仍与DI120相关(β=0.380,P〈0.001),而与DI0、DI30无明显相关性,说明血钾水平是DI120的独立影响因素。
结论在IGR中,血钾水平与胰岛β细胞功能(DI120)密切相关,从而可能为阻断IGR向2型糖尿病发展提供新的思路。
ObjectiveTo explore whether serum potassium can influence insulin secretion ability in patients with impaired glucose regulation (IGR) .Methods320 subjects with IGR were enrolled in our study. All subjects underwent a standard OGTT. Their clinical and biochemical parameters were measured. Insulin secretion indices were measured by DI0, DI30, and DI120.ResultsIn IGR, DI0 was negatively correlated with BMI (r=-0.122, P=0.015) , total cholesterol (TC) (r=-0.182, P=0.032) , low density lipoprotein-cholesterol (LDL-C) (r=-0.178, P=0.026) and triglyceride (TG) (r=-0.179, P=0.011) . DI30 was negatively correlated with TC (r=-0.146, P=0.001) , TG (r=-0.192, P=0.027) and LDL-C (r=-0.134, P=0.014) . DI120 was negatively correlated with TC (r=-0.222, P〈0.001) , TG (r=-0.209, P〈0.001) and LDL-C (r=-0.183, P〈0.001) while postively correlated with serum potassium (r=0.208, P〈0.001) . The multiple regression model analysis showed that serum potassium (β=0.380, P〈0.001) was independently correlated to DI120.ConclusionIn patients with IGR, serum potassium is associated with insulin secretion ability, which may provide a new treatment strategy for preventing IGR into T2DM.
