悬吊技术对脑卒中病人平衡功能的影响 被引量:6

摘要 目的采用重心摆动面积测定方法、表面肌电图(sEMG)以及平衡功能、运动功能评定量表等作为评价指标,观察悬吊技术(SET)对脑卒中病人平衡功能的影响。方法选取2014年6月—2015年6月在深圳大学附属第一医院接受治疗的脑卒中病人124例作为研究对象,按照病人在医院登记的就诊顺序随机分为对照组与观察组,每组62例。对照组给予功能训练、日常生活训练等常规干预,观察组在常规干预基础上给予悬吊技术。采用重心摆动面积测定方法、sEMG以及平衡功能、运动功能评定量表等,评价两组病人躯干控制肌群(左右竖脊肌、腹直肌)sEMG、运动功能、平衡功能、日常生活活动能力及生活质量。结果治疗前,两组病人Berg平衡量表(BBS)、Fugl-Meyer量表(FMA)、改良Barthel(MBI)、站立位重心摆动面积、左右竖脊肌和腹直肌sEMG值及SF-36量表评分比较,差异均无统计学意义(P>0.05)。治疗后,观察组BBS评分、MBI指数、FMA评分、站立位重心摆动面积、左右竖脊肌和腹直肌sEMG值及SF-36量表评分显著优于对照组(P<0.05或P<0.01)。结论相较于常规康复干预措施,悬吊技术可显著改善脑卒中后病人平衡功能,更有利于病人运动功能及生活能力的提高,进而改善其生活质量。 Objective To determine the effect of sling exercise therapy (SET) on the balance function in patients with stroke by usingdynamic balance analyzer,surface electromyography (sEMG),balance function and Fugl Meyer assessment (FMA).Methods Onehundred and twenty four patients with stroke in The First Affiliated Hospital of Shenzhen University from June 2014 to June 2015were randomly divided into two groups:control group ( n =62) treated with routine rehabilitation treatment,and treatment group ( n =62) treated with SET and routine rehabilitation treatment.The sEMG of trunk control muscle,motor function,balance function,the activi-ties of daily living (ADL),and the quality of life (QOL) were evaluated by gait analyzer,sEMG and the scales of Berg Balance Scale(BBS),FMA,Modified Barthel Index (MBI) and the MOS 36 item Short Form Health Survey (SF 36).Results There was no signifi-cant difference in the scores of BBS,FMA,MBI and SF 36,standing balance tests with open and closed eyes,the sEMG value of leftand right erector spinae and rectus abdominis between two groups before treatment (P 〉0.05).The scores of BBS,MBI,FMA,and SF 36,standing balance tests with open and closed eyes,the sEMG value of left and right erector spinae and rectus abdominis in the treatment group were better than that in the control group after treatment (P 〈0.05).Conclusion SET can also improve the balance function,motor function,daily life ability,and the quality of life in patients with stroke.
