四川省2016~2017年肿瘤患者感染细菌分布及药物耐药性监测分析 被引量:5

摘要 目的调查四川省肿瘤患者感染的细菌分布,分析细菌耐药情况,为临床的感染防治工作以及抗生素的合理使用提供依据。方法回顾性分析2016~2017年四川省细菌耐药监测网中,肿瘤科患者送检标本的细菌鉴定及药敏结果。结果送检的6861例标本中,共分离出细菌7825株。细菌的标本分布来源前3位为痰液(51.72%)、尿液(17.14%)和血液(12.24%)。分离细菌中,以革兰氏阴性杆菌为主,共5774株(73.79%)。占前3位的病原菌为大肠埃希菌(19.95%)、肺炎克雷伯菌(16.37%)及金黄色葡萄球菌(10.22%)。耐碳青霉烯类抗生素的大肠埃希菌、肺炎克雷伯菌以及耐甲氧西林金黄色葡萄球(MRSA)的检出率增加。结论四川省肿瘤患者感染的病原菌以革兰氏阴性杆菌为主。主要的病原菌耐药性呈现增长趋势。需加强肿瘤科室与微生物室的沟通,完善耐药监测的工作,指导临床合理用药,以减少肿瘤患者的危重感染,避免耐药菌的出现。 Objective To investigate the distribution and analyze the drug resistance of bacteria in cancer patients in our province in order to provide the evidence for infection control and rational use of antibiotics for these patients.Methods The identification results and drug sensitivity of bacteria in cancer patients during the period of 2016 ^2017 from Sichuan provincial bacterial drug resist-ance monitoring network were retrospectively analyzed.Results A total of 7825 strains of bacteria were isolated from 6861 specimens.The top 3 sources of bacteria were sputum (51.72% ) ,urine (17.14% ) and blood (12.24% ) .Among the isolates,gram-negative ba-cilli were the main strains (5,774 strains,73.79% ).The top three pathogens were Escherichia coll (19.95% ) ,Klebsiella pneumoniae (16.37% ) and Staphylococcus aureus (10.22% ).During 2016 -2017 ,the detection rates of Escherichia coli and Klebsiella pneumo-nia that resistant to carbapenem antibiotics, as well as the methicillin resistant Staphylococcus aureus ( MRSA) were increased when compared to before 2016.Conclusion Gram-negative bacilli are the main pathogens of cancer patients in our province.The drug resistances of the main pathogens show a growing trend.In order to reduce the critical infection of the cancer patients and avoid the emergence of drug resistant bacteria,it is necessary to strengthen the communication between the oncology department and the microbiology laboratory to improve the drug resistance monitoring and to guide the rational use of antibiotics.
