摘要
目的探讨人脐带间充质干细胞(hUC-MSCs)治疗心脏扩大伴心力衰竭类心肌病病儿的效果。方法心脏扩大伴心力衰竭类心肌病病儿17例,均接受hUC-MSCs治疗,分析治疗前后临床症状、体征及辅助检查结果。主要观察指标为病儿心力衰竭的症状,无症状生存时间,死亡和存活人数及左心室射血分数、左心室短轴缩短率,左心室舒张末期内径、左心室收缩末期内径和氨基末端脑钠肽前体(NT-proBNP)水平。结果 hUC-MSCs治疗后,17例病儿临床症状明显改善,活动耐力增强,13例(76.5%)病儿无症状生存;治疗后病儿左心室射血分数明显提高(P<0.05),NT-proBNP水平不同程度降低;15例(88.2%)病儿心功能不同程度好转或无恶化。细胞治疗后原发心律失常不同程度改善,生存质量改善,无死亡者。结论 hUC-MSCs治疗可改善心脏扩大伴心力衰竭的心肌病病儿的临床症状及心功能,改善生存质量。
Objective To investigate the clinical effect of human umbilical cord mesenchymal stem cells(hUC-MSCs)in the treatment of cardiomyopathy with cardiomegaly and heart failure in children. Methods A total of 17 children with cardiomyopathy with cardiomegaly and heart failure were enrolled and were given the treatment with hUC-MSCs.Clinical symptoms,signs,and auxiliary examination results before and after treatment were analyzed.Major observation indices included clinical symptoms of heart failure,symptom-free survival time,numbers of deaths and survivors,left ventricular ejection fraction,left ventricular fractional shortening,left ventricular end-diastolic diameter,left ventricular end-systolic diameter,and N-terminal pro-brain natriuretic peptide(NT-proBNP)level. Results After the treatment with hUC-MSCs,all 17 children had significant improvements in clinical symptoms and activity endurance,and 13 children(76.5%)achieved symptom-free survival.After treatment,the children had a significant increase in left ventricular ejection fraction(P〈0.05)and varying degrees of reduction in NT-proBNP level(P〈0.05).Of all children,15(88.2%)had varying degrees of improvement in cardiac function or had no deterioration.There were varying degrees of improvement in primary arrhythmia and a significant improvement in quality of life.No deaths were observed.Conclusion The treatment with hUC-MSCs can improve the clinical symptoms and cardiac function of children with cardiomyopathy with cardiomegaly and heart failure and improve their quality of life.
