脓毒症大鼠血糖水平与血清白细胞介素-10的关系

摘要 目的探讨盲肠结扎穿孔脓毒症大鼠不同时期血糖水平与白细胞介素-10(IL-10)的关系。方法 52只成年雄性SD大鼠按随机数字表法分为正常对照组(n=6)、假手术组(n=18)、脓毒症组(n=28),假手术组和脓毒症组再分为制模后6、12和18h共3个亚组。采用盲肠结扎穿孔术(CLP)制备脓毒症大鼠模型。酶联免疫吸附试验(ELISA)法检测各组不同时点血清IL-10、肿瘤坏死因子-α(TNF-α)水平;葡萄糖脱氢酶法快速检测尾静脉术前、术后6、12和18h不同时间点血糖水平,监测血糖变化;并对脓毒症组血糖水平与IL-10、TNF-α的相关性进行分析。结果与正常对照组相比,假手术组和脓毒症组大鼠制模前血糖、IL-10、TNF-α水平差异均无统计学意义(P均>0.05);与假手术组相比,脓毒症组大鼠制模后同时间点血糖、IL-10、TNF-α水平均升高(P均<0.05);脓毒症组大鼠制模后6、12和18h血糖水平逐渐降低(P<0.05),IL-10水平持续升高(P<0.05),TNF-α水平于12h升至峰值后随时间延长逐渐降低(P<0.05)。脓毒症组大鼠血糖水平与IL-10呈负相关(r=-0.740,P<0.01),与TNF-α呈正相关(r=0.538,P<0.01)。多元线性回归显示,脓毒症组大鼠血糖与IL-10呈独立相关(P<0.01)。结论脓毒症早期应激性高血糖是机体进化的一种防御反应,发病过程中应激性高血糖的波动与IL-10水平密切相关。 Objective To investigate the relationship between the changes of blood glucose and inflammatory immune factor IL-10 in rats with different periods of sepsis induced by cecal ligation and puncture. Methods Total 52 adult male SD rats were randomly divided into normal control group (n=6), sham operation group(n=18) and sepsis group(n=28). The sham operation group and sepsis group were divided into three subgroups: 6h,12h, 1Sh after modeling. The rat model of sepsis was established by cecal ligation puncture (CLP). Levels of serum tumor necrosis factor-oL (TNF-α ) and interleukin-10 (IL-10) were measured by enzyme-linked immunosorbent assay (ELISA). Blood samples were collected at 6h, 12h and 18h after operation to detect rapid blood glucose and monitor blood glucose changes. Glucose dehydrogenase assay was used to detect blood glucose levels at different time points of 6h, 12h and 18h from the end of the tail vein. Changes between blood glucose levels and peripheral blood inflammatory cytokines TNF- α , IL-10 in sepsis group were analysized by pearson linear correlation and linear regression analysis. Results Compared with the normal control group, there was no significant difference in the levels of blood glucose, IL-10 and TNF-α between the sham operation group and the sepsis group (P〉0.05). Compared with the sham operation group, glucose, IL-10, TNF- levels increased significantly (P〈0.05) in sepsis group after the mold of 6h, 12h, 18h. The level of blood glucose decreased(P〈0.05 ), and the level of IL-10 increased(P〈0.05 ), and the level of TNF- alpha was gradually decreased with time(P〈0.05) after the increase of 12h to the peak value. There was a correlation between blood glucose level and IL-10 and TNF- alpha in sepsis group(P〈0.01 ). Multiple linear regression showed that blood glucose and IL-10 were independently correlated in sepsis group (P〈0.01).
