摘要
目的探讨输尿管软镜钬激光碎石术治疗孤立肾上尿路结石的临床效果.方法 选择2016年1月至2017年12月收治的90例孤立肾上尿路结石患者根据手术方法分成两组,每组45例.对照组行传统手术治疗,观察组行输尿管软镜钬激光碎石术治疗.比较两组孤立肾上尿路结石清除效果,孤立肾上尿路结石清除时间、手术失血量、术后住院时间,手术前后患者肾功能指标中性粒细胞明胶酶相关脂质运载蛋白（NGAL）、血肌酐（Scr）和血尿素氮（BUN）、并发症发生情况.结果 观察组孤立肾上尿路结石清除效果高于对照组（P〈0.05）;观察组孤立肾上尿路结石清除时间、手术失血量、术后住院时间优于对照组（P〈0.05）;手术前两组肾功能指标NGAL、Scr和BUN比较发生率差异未见统计学意义（P〉0.05）;手术后观察组肾功能指标NGAL、Scr和BUN优于对照组（P〈0.05）.观察组并发症低于对照组（P〈0.05）.结论 输尿管软镜钬激光碎石术治疗孤立肾上尿路结石效果确切,可有效清除结石,减少创伤出血,降低并发症发生率,缩短手术时间,减轻对肾功能的影响,加速术后康复.
Objective To investigate the effect of ureteroscopic holmium laser lithotripsy on soli- tary upper urinary tract calculi. Methods From January 2016 to December 2017, 90 patients with soli- tary upper urinary tract calculi were divided into two groups according to different surgical method, with 45 cases in each group. Control group was treated by conventional surgery, and observation group under- went ureteroscopic holmium laser lithotripsy. The stone removal effectsbetween the two groups were com- pared . The isolated upper urinary tract stones average removal time, blood loss, average postoperative hospital stay were compared. The renal function indicators before and after surgery, including neutrophil gelatinaseassociated lipocalin （ NGAL）, serum ereatinine （Scr） and blood urea nitrogen （BUN） and com- plications were compared. Results The clearance effect of solitary upper urinary tract stones in observa- tion group was better than that in control group （ P 〈 O. 05 ）. The mean removal time of upper solitary urolithiasis, blood loss in operation and average postoperative hospital stay in observation group were bet- ter than those in control group （ P 〈 0. 05 ）. Before operation, the renal function indexes such as NGAL, Scr and BUN in the two groups were not significantly different （ P 〉 0. 05 ）. The renal function indexes such as NGAL, Scr and BUN in observation group after operation were better than those in control group （P 〈 0. 05 ）. The complications in observation group were less than those in control group （ P 〈 0.05 ）. Conclusions Ureteroscopic ureteroscopic holmium laser lithotripsy has good effect on patients with isolated upper urinary tract stones, which can effectively remove the stones, reduce traumatic hemorrhage and complications, shorten operation time, reduce the impact on renal function, and accelerate the postopera- tive recovery.
