摘要
[目的]观察麻杏甘石汤联合头孢曲松钠治疗急性成人社区获得性肺炎疗效。[方法]使用随机平行对照方法,将60例住院患者按病志号抽签随机分两组。对照组30例抗感染、止咳化痰,纠正水电解质紊乱,头孢曲松钠,80mg/kg·d^(-1)+10%葡萄糖150mL,静滴,2次/d。治疗组30例麻杏甘石汤(麻黄~炙、杏仁、茯苓、白术、地龙各10g,石膏~生30g,甘草~炙5g,~川贝母3g,严重痰热加黄芪;肺阴虚加沙参;痰多加鱼腥草;肺气虚加黄芪)。1剂/d,水煎30mL,分早晚两次服用。连续治疗1周为1疗程。观测临床症状、症状消失时间、血清CRP、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效22例,有效6例,无效2例,总有效率93.30%;对照组显效16例,有效5例,无效9例,总有效率70.00%;治疗组疗效优于对照组(P<0.01)。血清CRP改善治疗组优于对照组(P<0.05)。体温恢复正常时间、咳嗽咳痰消失时间、肺部啰音消失时间治疗组低于对照组(P<0.05)。[结论]麻杏甘石汤联合头孢曲松钠治疗急性肺炎,疗效满意,无严重不良反应,值得推广。
[Objective] To observe the curative effect of Maxing Ganshi decoction combined with ceftriaxone sodium in the treatment of acute pneumonia. [Methods] using random parallel control method, 60 hospitalized patients were randomly divided into two groups according to the number of disease records. Control group 30 cases of routine anti infection, cough and phlegm, correction of water and electrolyte disorders, ceftriaxone sodiumintravenous drip, 80 mg/kg · d-1＋10% glucose intravenous drip 150mL, 2 time/d. In the treatment group, 30 cases of Maxing Ganshi decoction （ephedra grilled, almond, poria, atractylodes and dragon 10g, raw gypsum 30g, licorice 5g, bfritillaria 3g, severe phlegm heat plus Huangqi; lung yin deficiency and sand cucumber; phlegm and Houttuynia; lung Qi deficiency plus Huangqi）. 1 doses of/d, deeoet 30mL, and take it two times in the morning and evening. Continuous treatment for 1 weeks was the 1 course of treatment. The clinical symptoms, symptoms disappeared time, serum CRP and adverse reactions were observed. The 1 course of treatment was treated and the curative effect was determined. [Results] 22 cases were found in the treatment group, 6 cases were effective, 2 eases were invalid, the total effective rate was 93.30%, the control group was 16 cases, 5 cases were effective, 9 cases were invalid and the total effective rate was 70%. The curative effect of the treatment group was better than the control group （P〈0.05）. The improvement of serum CRP in the treatment group was better than that in the control group （P〈0.05）. The recovery time of body temperature, the time of cough and expectoration disappeared and the time of pulmonary tales disappeared in the treatment group were lower than those in the control group （P〈0.05）. [Conclusion] The curative effect of Maxing Ganshi decoction combined with ceftriaxone sodium in the treatment of ＇acute pneumonia is satisfactory, and there is no serious adverse reaction. It is worth promoting.
