摘要
目的探索不同浓度丝裂霉素C对体外培养的人翼状胬肉细胞的影响。比较联合自体角膜缘移植手术与常规自体结膜瓣移植术的优越性。方法体外培养原发翼状裔肉10例,无菌条件下培养胬肉组织,对培养的胬肉细胞进行CD34、CD44表型鉴定,经不同浓度丝裂霉素C作用于培养的胬肉细胞后,酶标仪检测10天内各组细胞吸光度,经选择最适浓度的丝裂霉素C作用于翼状胬肉患者,联合自体角膜缘移植手术(limbal conjunctival autograft transplant,LCAT)治疗翼状胬肉患者53例,与常规的翼状胬肉术式自体结膜瓣移植术(conjunctival autograft transplant,CAT)50例进行对比。结果原代培养后可得到人翼状胬肉成纤维样细胞,经传代纯化后流式细胞仪检测细胞不表达CD34,高表达CD44,0.2 mg/m L的MMC作用于翼状胬肉细胞5min其细胞活性最稳定,丝裂霉素C联合LACT术后随访,完成随访49例49眼,术后复发2眼,复发率4.1%。CAT完成随访45例,术后复发4例,复发率8.9%。两组间复发率有统计学意义(P<0.05)。结论本实验培养获得了人翼状努肉成纤维细胞,选择最适丝裂霉素C浓度处理联合LCAT与CAT均能治疗原发性翼状胬肉,降低术后复发率,但LCAT联合丝裂霉素是治疗原发性翼状胬更优术式。
Objective To explore the different concentration mitomycin Con the influence of human pterygium cells in vitro culture.Compared with the advantages of autologous corneal limbus transplantation surgery with conventional autologous conjunctival flap transplantation. Methods In vitro culture of pterygium 10 cases, aseptic conditions to develop pterygium organization and to identify the phenotype of CD34, CD44 cells, in different concentration mitomycin C to cultivate pterygium cells, each cell enzyme standard to test 10 days absorbance, selected the optimal concentration of mitomycin C on pterygium patients, autologous corneal limbus transplantation surgery(limbal conjunctival autograft transplant, LCAT) 53 patients with pterygium treatment, with conventional pterygium surgery autologous conjunctival flap transplantation(conjunctival autograft transplant, CAT) 50 cases were compared. Results The cultured primary generation cells can be obtained from pterygium into fiber sample cell, after generation of flow cytometry instrument testing cells don't expression CD34, high expression of CD44, 0.2 mg/mL of MMC on 5 min the pterygium cells activity the most stable, mitomycin C combined LACT postoperative follow-up, complete followup 49 cases(49 eyes, 2 eyes of postoperative recurrence, the recurrence rate was 4.1%.CAT complete followup of 45 cases, 4 cases of postoperative recurrence, the recurrence rate was 8.9%. The recurrence rate between the two groups was statistically significant(P〈0.05). Conclusion The experimental training receives one pterygium fibroblasts, selects the optimal concentration of mitomycin C treatment combine with LCAT CAT can treatment of primary pterygium, reduces the postoperative recurrence rate, but LCAT joint mitomycin is more optimal surgical treatment for primarypterygium.
